Government / agencyIndexed Oct 16, 2026Source-linkedVerify source — SaferProducts.gov incident

SaferProducts.gov consumer incident report 20250730-8B670-2147329079. Severity: Incident, No Injury. Product: OKLEEIC 804 (OKLEEIC 2 Prong to 3 Prong Outlet Adapter, 14 in 1 two prong to three prong power strip. It has 8 three prong AC ports, and 6 usb ports (3 usb-C and usb-A) in white. 2 prongs where it plugs into the wall but allows for 3 prong items to be plugged into it. the email on the user manual is oleeic@163.com). Incident: I bought a surge protector on Amazon that has only 2 prongs to go into an outlet but accepts 3 prong "grounded" plugs. This seemed off to me and other [REDACTED] users advised me not to use it. Most people responded by basically saying not to use it or to not plug in 3 prong items into it. Admittedly I didn't read the description carefully enough to realize it was a 2 prong item. I purchased it because it has 3 usb-C ports to charge my electronic devices. I wanted to report this item because it seemed like a potential hazard, and many people on [REDACTED] advised me that it was a fire risk. my [REDACTED] user name is [REDACTED] and I made the post on the 29th of July. T. Nothing dangerous has happened to me while I was using it, but I'm not willing to take the risk. To be clear, I only used it very briefly and only whilst sing the usb-c port. Total use time was less than 10 minuets. If you are interested in the social media post that supported my concern; my [REDACTED] user name is [REDACTED] and I made the post on the 29th of July 2025.
