Incident narrative

SaferProducts.gov consumer incident report 20110618-092A3-2147478091. Matched catalog product: HP HP Pavilion 17 Notebook PC. The product number is G4X63UA#A. Source model text: PPP012L-E. Incident: I ordered a laptop charger, model # HP Compaq AC Adapter 19V 4.74A 90W PPP012L-E from an Amazon.com seller - Triangle World. The Amazon order number was [REDACTED]. The charger arrived June 11th, and I have been using it for one week. Today, I smelled smoke, the charger had overheated and failed, and is visibly discolored and slightly warped. This unit is a potential fire hazard and may be a potential counterfeit product. I filed a claim with Amazon and let them know I would be filing a complaint with the CSPC.
