Incident narrative

SaferProducts.gov consumer incident report 20120620-B7DD4-2147466420. Severity: Incident, No Injury. Matched catalog product: Graco DuetSoothe Swing and Rocker - Model 1893776. Source model text: 9B08CAT. Incident: I have a Graco Playard model # 9B08CAT manufactured in 2/2010 and in use since 1/2011 (~16 months). When using the bassinet portion of the playard for my 4 month old son, he began crying like he was hurt. A different cry than usual. I went into the bedroom to check on him and pick him up and he was stuck. I couldn't physically pick him up. I then realized his head was stuck under the oval wall part of the bassinet and the 4 panel mattress has bunched up (the 4 panels overlapped each by about 1/2 inch - 1 inch) to create a space at the end of the mattress. I wanted to report this so that if there other similar reports, a safety recall could be issued to prevent future injuries. I believe the problem is that the fabric of the mattress has stretched a bit over the 16 months of usage and now allows the 4 boards inside the mattress to overlap a bit which creates space at the edges of the bassinet. Because the bassinet has walls that shape the space into an oval, the oval wall can trap a baby's head. My mattress also with 16 months of use has bo
