Incident narrative

SaferProducts.gov consumer incident report 20171028-DB6CA-2147396391. Matched catalog product: KitchenAid KDTM354DSS4. Incident: Please be advised that the top rack failed this morning almost with catastrophic results. The washer completed its cycle and the top rack was being pulled out for emptying when without warning the right side gave way. Luckily for Kitchenaid, my 71 year old reflexes held onto the rack preventing it from spewing glass on our new kitchen ceramic floor. The only injury was a slight blood prick and abrasion to the side of the middle finger. Was able to gingerly empty the rack free of glassware and ceramic cups without incident. Observation of failure area showed that the two side rail support lugs failed one apparently by straight shear and the other actually pulled out from the plastic bracket. Was able only to salvage the pulled out lug. Note that this washer is a replacement for a Kitchenaid Dishwasher that had failed due to a severely overheated circuit board. Filed all of the paperwork and yet to receive any monetary compensation from this call action suit. The washer had been installed in May 2016.
