Incident narrative

SaferProducts.gov consumer incident report 20180404-96CCD-2147388812. Severity: Incident, No Injury. Product: Anchor Hocking It exploded. I do not know (Anchor Hocking Casserole 4 quart). Incident: I prepared lemon chicken as usual. I have made this dish for over 15 years the same way. I placed the chicken in the oven in an Anchor Hocking glass casserole dish. Five minutes later I heard an explosion in my kitchen and smoke began to fill the oven, kitchen, living room and family room. I immediately turned off my gas stove. When I opened the door of the oven, I saw that the glass casserole dish had exploded in my oven. There was olive oil and chicken broth and broken glass all over the bottom of my oven. The 5 pieces of chicken were sitting on the remainder of broken glass on the oven rack. This is a Fire Hazzard! I can only imagine if I had not been in a room close to the kitchen when this happened. My whole home could have burned down. Not to mention, if my teenager or any family member had removed this faulty casserole dish from the oven with the defect that HAS BEEN KNOWN to THIS company for years, a severe oil burn, or laceration could have happened. There have been many complaints regarding this same issue with Anchor Hocking. I find it unusual that there has been nothing done about these SAFETY issues with HUNDREDS of Complaints. ANCHOR HOCKING has explained the exploding glass as a safety mechanism or faulty care by the consumer. My casserole was less than 1 year old. There was NO extreme temperature changes and NO damage to my casserole prior to using. I now am afraid to use my Kitchen Aide oven that is saturated with oil near the gas igniter. I have saved the glass from the casserole dish for testing if needed. I also have many pictures.
