Incident narrative

SaferProducts.gov consumer incident report 20190103-CE27D-2147383428. Severity: Incident, No Injury. Product: Black and Decker Convection Countertop oven TO1640B (Black and Decker Countertop Convection Oven 12" pizza capacity). Incident: In June of 2018, I replaced my former Black and Decker digital convection toaster oven with this new, out of the box, Black and Decker countertop convection toaster/oven (Model TO1640B). I purchased this current oven at BIG LOTS on North 72nd St in Omaha, NE in 2013. (I stored it a few years because I thought my former toaster oven was failing, but discovered a button stuck on it, and once that was corrected the oven lasted another 5 years). After some months of using this current oven, it became apparent that there was some design flaws that gave me concern of its safety and potential fire hazard. Two main problems have occurred repeatedly over the past few months, and it wasn't until recently that the oven was discovered ON and Very hot after having supposedly shut off over an hour prior. This occurs in one of 2 ways: 1. If you are wanting to toast or heat and item for less than 10 minutes (as in a 2-3 minutes to toast an item and fail to turn the dial for the toast/timer far past 20 and then counter clockwise to the 2-5 minutes, the unit will stay on at whatever temperature the dial setting for TEMP is at, and will remain on for an infinite time. (this happened most recently on Sunday December 30, 2018, though it has happened on other occasions prior). with this incident an piece of bread still remained in the oven and upon smelling the burning some long hour or so later, it became evident the oven was still ON. Even though the appliance has a note to turn past 20 before setting the time, this seems haphazardly dangerous and has no safety measure to assure an operator ha
