Incident narrative

SaferProducts.gov consumer incident report 20210708-E0A41-2147363060. Severity: Injury, Emergency Department Treatment Received. Matched catalog product: Uppababy 2017 Uppababy Cruz. Model Number 0417-CRZ-US. Source model text: Rumble Seat (Model Number: 0101). Incident: My husband was pushing my Two year old daughter in the UPPAbaby Vista stroller. She was sitting in and strapped into the Rumble Seat (model number 0101, manufacture date Aug. 2014). The stroller hit a bump in the sidewalk and the Rumble Seat adapters detached from the stroller, ejecting the seat holding my daughter. She landed face first onto the sidewalk. Her forehead split open in two places and we had to take her to the emergency room where she got ten stitches. She also got surface scrapes on both wrists, one knee, and one toe. Five weeks later (today) she has large scars on her forehead where the stitches were.
