Incident narrative

SaferProducts.gov consumer incident report 20220404-A2EFF-2147357005. Severity: Incident, No Injury. Matched catalog product: Graco DuetSoothe Swing and Rocker - Model 1893776. Source model text: Model: 2140059 name: sr35 lt. Incident: Purchased the Graco FastAction SE Travel System for my newborn baby. I was walking my 4 month old in this system. He was in the car seat, strapped in I went over a small crack in the road and the stroller completely folded upward in its stow away position. The stroller fell with the car seat still attached to the stroller. The car seat went down head first. My baby did not hit the ground but the car seat hit the ground pretty hard. When I got home I called the pedi to check in on my son, and report the issue with graco. There many reports online reporting this same exact issue. Not sure why product has not been recalled. I am no longer using this system.
