Incident narrative

SaferProducts.gov consumer incident report 20221003-74B16-3925792. Severity: Incident, No Injury. Product: Libbey Gibraltar Tumbler Glasses, 16-ounce (Libbey Gibraltar Tumbler Glasses, 16-ounce, drinking glass). Incident: Safety report: Product: Libbey Gibraltar Tumbler Glasses, 16-ounce, drinking glass. Manufacturer: Libbey, 300 Madison Avenue, Toledo, Ohio 43604 Incident report: On September 18, 2022 my three year old daughter was holding a Libbey brand drinking glass in her hands. She had lime juice with a few ice cubes, in her glass. My daughter did not drop the glass, and she was holding it normally while watching TV. However, for unknown reasons. the glass broke apart and basically "exploded" to some extent, while my daughter was holding the glass. There were no visible cracks in the glass. The glass was purchased on March 23, 2022 directly from the manufacturer Libbey. The glass has "Duratuff' on the bottom, indicating it's "tempered glass," and thus should be stronger. Again my daughter did not drop the glass. She was, simply holding it. Yet it broke apart in her hands. Photos are enclosed. Child affected: [REDACTED], date of birth [REDACTED], my daughter living with me and my wife and other children. Sincerely, [REDACTED]
