Incident narrative

SaferProducts.gov consumer incident report 20230313-650FE-2147348794. Severity: Incident, No Injury. Product: Lennox Heat Pump Package Unit Model 13HPP42AP-3A (Lennox Heat Pump Package Unit Model 13HPP42AP-3A serial number 1614K06686.). Incident: I purchased a package heat pump unit Model 13HPP42AP-3A on 11/11/14 unit serial number 1614K06686. Unit has been serviced regularly. On 2/3/23, unit air handler fan would not shut off and system would not run in automatic or on emergency mode. Eventually I was able to get the system to run in emergency but air handler would not shut off. Contacted the installing dealer and the unit was repaired on 2/4/23. The control wires for the unit were wired tightly over an exposed metal edge with no stand offs or grommets to prevent wire wear. Vibration off the unit during operation damaged the insulation on the wires causing a short circuit that resulted in an energized cabinet as well as failure of the 40 VA Universal Transformer and Double Pole Breaker. This is very obviously either a manufacturing problem or a serious design flaw. This could have resulted in a house fire or an injury to someone.
