Incident narrative

SaferProducts.gov consumer incident report 20250902-B413D-2147328147. Severity: Injury, First Aid Received by Non-Medical Professional. Product: OneWheel OneWheel GT-s (In description.). Incident: Product: OneWheel GT-s Manufacturer: Future Motion, Inc. Serial Number: OW452872 Hardware: 8411 Firmware: Polaris - 6208 Date of Incident: [Insert Date] Total Mileage: 55 miles (nearly new) Incident Description: Our OneWheel GT-s appears to have a serious safety defect related to its battery and controller. The unit charges only to 90% capacity, according to the app. After charging, when my son attempted to ride, the board initially struggled to engage balance mode. Once it did, within 3040 seconds, the unit began to beep with a low battery warning, despite showing 90% charge. It then abruptly pitched back and forced a complete stop. We attempted to reset the charge by leaving it on the charger for a full day, verified the 90% reading again, and repeated the ride. The exact same failure occurred this time causing my son to fall and sustain injury due to the sudden stop. At the time of the incident, diagnostics reported the following: Battery Temperature: 75.2F Controller Temperature: 80.6F Motor Temperature: 73.4F Hazard: The unit appears to falsely indicate sufficient charge while simultaneously triggering a shutdown protocol within minutes of use. The sudden, unannounced stop creates a high risk of serious injury to riders. Outcome: My son was injured when the board abruptly powered down. The unit is unsafe and cannot be used. Requested Action: We are requesting that the CPSC investigate this safety defect, as it may be systemic across the GT-s product line. The hazard mimics previously reported OneWheel nosedive incidents that have led to severe injuries and fatalities.
