Incident narrative
SaferProducts.gov consumer incident report 20250912-51FDC-2147327883. Severity: Incident, No Injury. Product: Strikeman Strikeman Laser Cartridge .40 S&W (The Strikeman laser Dryfire Firearm Training System UPC: 850048771253, Model SYS-DRI01 is a system that allows target type practice by replacing firearm ammunition with a laser emitting cartridge, a permanent target and cellphone App to determine where the laser pointed to when the trigger is pulled. My complaint is about the .40 S&W Laser Cartridge LSR003, UPC: 850048771024). Incident: I purchased a Strikeman Laser Firearm Training System with a .40 cal laser module. The laser module uses rubber O-Rings for retention and centering in the barrel. I tested the system, then I removed the laser module from the firearm, and noticed the front O-Ring was missing from the module. I checked the barrel. I found the O-Ring was stuck in the barrel, creating a blockage that could have resulted in a catastrophic failure and personal injuries. I contacted the company and have not gotten a satisfactory response and followup, which is shocking, considering what could happen if a live round is used afterwards when a blockage exists. I was able to duplicate the problem and even sent in pictures to the manufacturer. I explained the engineering team needs to be notified and there may be a manufacturing defect where the channel is not deep enough to hold the O-ring in place as the module is removed from the barrel.
