Incident narrative

SaferProducts.gov consumer incident report 20251006-3C0CF-2147327295. Severity: No Incident, No Injury. Product: Dr.Browns Dr. Brown's HappyPaci 100% Silicone Baby Pacifier, (100% Silicone Soild pattern). Incident: Consumer Product Safety Report: Product: Dr. Browns HappyPaci 100% Silicone Pacifier Manufacturer: Handi-Craft Company (Dr. Browns) Retailer(s): Amazon (online) Incident Date: 10/4/2025 Consumer Location: [REDACTED] Description of the Incident On 10/4/25 I experienced a serious product failure involving a Dr. Browns HappyPaci 100% Silicone Pacifier. My infant was 4 weeks and 1 day old at the time of the incident. The product had been in use since my baby was born. At that time, I assumed this was normal wear or possibly caused by a household pet, and I discarded the damaged pacifiers. All cleaning and sterilization were done according to the manufacturers directions. I washed the pacifiers only by hand using [REDACTED] Baby Bottle/Dish Soap and [REDACTED] Pacifier Wipes, and I sterilized them every 3 to 4 days in a Dr. Browns sterilizer, placed on the top rack as indicated in the manufacturers instructions. The water used for cleaning was warm (approximately 100F), not boiling or scalding. My baby had over 24 Dr. Browns HappyPaci pacifiers, so each individual pacifier received minimal use. The product was labeled as durable and easy to clean on both Amazon and [REDACTED] listings. During the incident, while handwashing one of the pacifiers, I noticed a cat hair on the nipple tip (the bulb). When I lightly pulled the hair away using my fingers, the entire bulb detached cleanly from
