Incident narrative

SaferProducts.gov consumer incident report 20260217-E10BB-2147323477. Severity: Injury, No First Aid or Medical Attention Received. Product: HTC VIVE VIVE Tracker (3.0) Dongle Cradle (The product is the VIVE Tracker (3.0) Dongle Cradle (Model: 54H20704-00P). It is a compact, weighted, black plastic docking accessory included in the VIVE Tracker 3.0 ecosystem. Dimensions are approximately 1.5" (L) x .5" (H) x .5" (W). The device features a single USB-C female input port on the rear for power/data and a single USB-A female output port on the top designed to host a wireless receiver dongle. Safety & Identification Hazards: > The device is a "sterile unit" with no external serial numbers, SKU markings, or date codes etched or printed on the housing. This lack of traceability is a primary safety concern. The SKU provided (99HASS001-00) and UPC (821793052546) are for the parent retail kit, as the failed cradle itself has no individual identifiers. This prevents consumers from identifying specific defective batches or production runs prone to the thermal runaway reported here. Incident Summary: The unit suffered a catastrophic thermal failure at the USB-C input port, resulting in melting and the release of toxic fumes. The manufacture date (estimated March 2021 based on retail launch) cannot be verified due to the manufacturer's failure to include a physical date code or batch number on the power-conducting hardware. This lack of a Manufacturer Date Code is being reported as a secondary safety violation.). Incident: On February 13, 2026, a VIVE 3.0 Dongle Cradle experienced a catastrophic thermal runaway event while connected to a standard PC workstation. Under normal use, this device is designed to be powered constantly. The USB-C housing suffered confluent melting and internal pin charring. Notably, my motherboard remains undamaged and shows no signs of charring, providing clear evidence that the failure originated within the VIVE hardware. Upon detecting a strong burnin
