Incident narrative

SaferProducts.gov consumer incident report 20260622-BF9E8-2147315735. Severity: Incident, No Injury. Product: Novaic G-R-48150 (Novaic G-R-48105 LIFePo4/ Lithium ion battery). Incident: Respondent Business Details:Company Name: [Insert Battery Brand or Seller Name]Website/Platform: [e.g., [REDACTED], or Seller Website]Order Number: [REDACTERD]Details of Complaint:I am a Michigan resident reporting a severe product safety hazard and consumer fraud. I purchased a high-voltage battery from the respondent. Upon delivery and subsequent technical testing, I discovered a critical manufacturing defect: a severe insulation fault. Testing indicates 44V is leaking from the positive terminal to the exterior metal case, and 15V from the negative terminal to the case. This renders the outer shell of the battery electrically live. It poses an immediate risk of catastrophic electrical shock to anyone who handles it, alongside an extreme fire and short-circuit risk.I reached out to the seller's customer service department to invoke my warranty rights for a replacement or refund. On June 19, an agent named [REDACTED] explicitly rejected the safety risk, stating in writing: "the insulation fault does not affect use of battery. you can use it without problem."Advising a consumer to continue operating a high-voltage lithium battery with a live metal chassis is incredibly negligent, violates basic product safety rules, and endangers my household. The company is actively refusing to honor their warranty obligations for a dangerous item. I have disconnected and isolated the battery to prevent a fire.Relief Requested:I am seeking a full refund for this defective and dangerous item. Furthermore, because a battery with an insulation fault cannot be safely packaged and shipped back v
