Incident narrative

SaferProducts.gov consumer incident report 20260915-56547-2147309938. Severity: Incident, No Injury. Product: Kohler Levity glass shower door Model # 706165-L-NX (Kohler Levity Frameless Sliding Shower Door (Brushed Nickel, 59.625 in. W x 82 in. H). The door was purchased from Home Depot in April 2023 and installed in June 2023. The delay in installation was due to product flaws requiring us to reorder the doors multiple times we received three sets of doors (6 doors) to be able to identify two without visible manufacturing flaws and had to incur the cost of transporting the flawed doors to the dump for disposal. We sent Kohler photos of the flawed doors. The doors were finally installed after the third set was received and here is the information on the doors: Model # 706165-L-NX Serial # SAU3NNNDF 06/08/2022 Model # 1381836-NA-GC05 Serial # SAU3PNDJ6 08/18/2023 Model # 1381837-NA-GC05 SAU3PLWC3 07/26/2023). Incident: Spontaneous Glass Explosion - Kohler Levity Shower Door (706165-L-NX) We have experienced a severe product failure involving our Kohler Levity Frameless Sliding Shower Door (Brushed Nickel, 59.625 in. W x 82 in. H). The door was purchased from Home Depot in April 2023 and installed in June 2023. The delay in installation was due to product flaws requiring us to reorder the doors multiple times we received three sets of doors (6 doors) to be able to identify two without visible manufacturing flaws. While we were away from 09/10/2026-09/13/2026 and no one was within the home, one of the tempered glass doors spontaneously exploded. There was absolutely no impact, misuse, or physical contact. Glass fragments scattered throughout the bathroom, embedded in the ceiling, and projected into the hallway. Given these specific safety circumstances, we are requesting a refund of the original purchase price from Kohler instead of a physical replacement door.
