FUEL/PROPULSION SYSTEM

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11657124

When filling up with gas, the gas filler tube would overflow spilling gas out of the filler tube. This happened multiple times. The dealer replicated the issue and found that the back flow sensor, valve and back flow door in the gas tank were broken and faulty. They replaced the gas tank with all of the above components replaced as part of the gas tank.

Passenger side headlight was making a loud noise. When it began, it happened sporadically, but in ~two weeks, it happened consistently whenever the car was on and could often be heard while driving. The sound was consistent with other reports of the headlight assembly failure and those covered by TSBs for 2019–2023 models. The vehicle was brought to the dealership, the noise was verified as being the headlight failure, and it was replaced under warranty.

While driving on the freeway, the car stopped having the ability to accelerate. The engine light came on, and I received a notification on the Mazda app stating there was an "engine malfunction." I took it to Mazda, and they told me nothing was wrong. Flash forward a month and the same exact situation occurred. I took it to Mazda, and they told me the same thing. This is incredibly dangerous, as losing the ability to accelerate on a very busy freeway is life-threatening.

The side mirror defrosters are not operable. This is an extreme safety hazard to be able to see my surroundings while driving. This function should operate when the rear defroster is activated and is not working. I cleared the side mirror prior to leaving my house, during driving the mirror frosted back over causing me to not see other vehicles. The alert when a car is next to me would not function due to ice build up. If a crash were to happen due to the safety features not properly working, this could cause issues. This seems it should come standard on all Mazda vehicles considering there safety satisfaction. This is the third Mazda I've owned and I've never had a complaint until now.

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11591218

I had coplained about a long, soft brake pedal on 10 February 2024 to Hiley Fort Worth Motorcars and got a report on 13 February 2024 stated that after inspection, they could not duplicate my complaint, so they did a visual inspection and washed the car!!!! Today, 29 May 2024, I had to make an emergency stop and pressed the brake pedal to the floor! The ABS came on, but the brake pedal should never reach the floor. The other car had stopped and I swerved out of the way. No warning lights came on nor sounds. Additional information; if the brakes are applied by the adaptive cruise control, and you step on the brake pedal after the brakes have been applied automatically, the brake pedal is nice and firm, like it should be. But if there is no intervention with the car, the brake pedal is very long and soft.

This vehicle belongs to my friend, but I was the injured party. She was visiting me and realized she forgot her phone,.the vehicle wias at the top of my driveway and in park. She wanted to go back in the house to look and got out of the car & left the door open., I leaned into the car to press stop engine and immediately the car began to roll down the hill, dragging me 50'. Car still in park slammed into the stone wall of my garage.I had injuries to my right leg. aplit lip, frctured nasal bone, nerve damage to my thigh and torn meniscus. The Ambulance arrived, I was taken to the hospital and treated. Witnesses signed affadavits confirming vehicle still in Park. I have not been awarded any compensation since they say that my injuries were not serious enough.Mazda,at this time, has refused to take respomsibility since they could not duplicate the incident remotely tested by a Calif.facility. I am reporting this incident because all other parties have failed to do so.

SERVICE BRAKES,FORWARD COLLISION AVOIDANCE

The most recent incident: Driving 35 MPH on a 4 lane divided boulevard, vehicle 5-6 car lengths in front of me indicates intention to turn right. No traffic in lane to my left should I need to change lanes. I continue to coast. At approximately 2-3 car lengths, the vehicle in front has nearly completed the right turn - there is no risk of collision - but my car BRAKES HARD. This is the 4th time my car has activated emegency braking in a situation it is not warranted. I have been dirving for 36 years with no accident. I do not need nor want this technology as it is more prone to causing accidents than preventing them. Additionally, the car throttle response is slow to recover in these situations, so while I mashed the acclerator to the floor to avoid being rear-ended - the car did not accelerate for 1-2 seconds (RPM went up, but the engine did not engage for several seconds). In situations where AEB might have been marginally helpful, detecting cross traffic running red lights, the car has NEVER detected this - and thankfully I'm an experienced driver so AEB was not needed. This technology, IMO, is not safe and will lead to over-reliance causing more accidents in the future.

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11762181

Component failure: potential thermal (heat-related) breakdown of the EPS control module, steering rack electronics, or associated wiring harness. Description: After approximately two hours of continuous highway driving, a critical Red Steering Wheel Warning Light illuminates intermittently in the center dashboard. This indicates a severe system fault and imminent threat of total loss of power steering assist while operating at highway speeds presenting an extreme safety hazard. The vehicle is under the bumper-to-bumper warranty and was brought to authorized Mazda service for the warranty repair. The dealership refused to perform a deep electronic diagnostic of the EPS module. The service manager released the vehicle back to me, claiming the issue cannot be duplicated unless it is driven for two hours and no active codes are showing. He is claiming this is a lane assist issue and the vehicle is safe. This is not a lane assist issue as it was occurring for the complete two hour trip and not on a specific road area that the lane assist could not detect. By refusing to escalate a critical steering failure to manufacturer engineers, the dealership has forced me to continue driving a potentially unsafe vehicle with a known steering defect in order to “prove” the malfunction occurs.

Straight to the point, I backed the vehicle up, pushed the brake peddle in and shifted into park. The engine begun to increase in RPMs and I could hear the engine load increase, maybe about 2,000 to 5,000 rpms higher than idle or first start building vacuum idle, as if I was pushing on the accelerator. I literally looked down at the peddles, as I was just doing a quick spot change in my driveway, the accelerator did not appear to be stuck or in contact with the aftermarket winter weather plastic floor mat. I decompressed the brake peddle again and shifted the car from P to D and the engine went to its normal baseline idling RPMs and sound. I put it back in P and turned it off, no issue. I have noticed this problem before, but I only noticed it during this past winter season and blamed it on my winter boots and/or myself instead of the vehicle. I am going to guess this has happened in total about 4 times, including the most recent incident that I just wrote about. The CX-5 is connected to Mazda's network/ data service or whatever it is called, because I like my cars clock to maintain correct time, which it won't or can't do if I prefer my privacy from Mazda. So Mazda should be able to see what happened on their end. It happened in the late afternoon to early evening. I do also use 'auto hold' daily and cruise control w/out radar active.

I bought this car brand new with 51 miles on it on 7/24 and have taken it to the dealership mechanics twice for the same problem. Within 3 seconds of “letting go” of the steering wheel, the car veers and pulls directly into the right berm. The car requires continuous redirection of the steering wheel to get it to drive straight on. The dealership where I purchased this has not reported this defect to the manufacturer despite me requesting it. The car was inspected prior to me purchasing it, and upon me noticing that it veers off the road, I took it back to the dealership garage (on 7/31) and the mechanics reported that they had rebalanced and realigned the tires. The mechanics also told me they had realigned the front and rear axles and said that they had probably been affected by being strapped down tightly on the delivery truck (?). I picked up the car the same day (7/31) after they said they fixed it, and within 10 miles of leaving the dealership, I noticed that the car continued to pull me off the road to the right. I turned around and returned to the mechanics’ and requested they report this to the manufacturer. The car has remained at the dealership mechanic’s since 7/31. No warning lights or messages have ever shown on the car. I consider this a major safety hazard as the car is constantly and consistently veering either off the road into the berm, or into the right lane if attempting to pass.

AIR BAGS,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11745244

On 06/18/2026, the vehicle displayed multiple safety warnings, including ABS malfunction, dynamic stability control/traction control malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and airbag/seat belt pre-tensioner malfunction. These warnings involve braking, stability control, collision avoidance, airbag, and seat belt safety systems. This was not the first occurrence. On 04/24/2025, the vehicle displayed similar warnings, including critical brake system/Electronic Brake Force Distribution malfunction, ABS malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and keyless entry malfunction. The vehicle was towed at that time. The vehicle was inspected by an authorized Mazda dealer in June 2026. The dealer replaced the battery under warranty, but I was not provided diagnostic trouble codes, a root-cause explanation, or written confirmation that the braking, ABS, airbag/seat belt, stability control, Smart Brake Support, Forward Obstruction Warning, and forward sensing/camera systems were fully diagnosed and verified safe. The dealer inspection report also listed the mileage as “No Mileage.” The vehicle is available for inspection upon request. I do not feel safe driving it because the repeated warnings involve braking, collision avoidance, stability control, airbags, and seat belt systems. I am concerned that an electrical, braking, or safety-control defect may disable safety systems while driving.

Sunroof glass failure that occurred on my 2024 Mazda CX-5. While driving to work on May 9th at 11:16 pm going 67 miles per hour on route XXX in [XXX] , I suddenly heard a very loud popping/explosive sound from above the vehicle. Upon safely pulling over, I discovered that the sunroof glass had shattered unexpectedly. There was no known object strike that I or my two passengers observed, no prior cracks or chips visible before the incident, and no warning signs of damage beforehand. The glass appeared to have exploded outward/ upward and appeared curved upward. Most of the glass we believe hit the vehicle behind us as it was gone by the time we were able to safely pull over. It was approximately 58 degrees outside with no inclement weather. The vehicle currently has approximately 53,000 miles and has been properly maintained. The incident was extremely startling and created a significant safety concern while driving. Luckily, the interior sun visor was closed when this incident occurred, preventing the glass from entering the vehicle and injuring myself or my two passengers. Because the vehicle is relatively new and the damage appeared sudden and unusual, I would sincerely appreciate Mazda reviewing this matter for possible warranty or goodwill assistance. I have always valued Mazda’s quality and reputation, and I am hopeful the company can help support resolution of this unexpected issue. I have attached photographs of the damage and would be happy to provide any additional information needed. I would also appreciate the dealership documenting whether any definitive impact point or external strike evidence was identified during inspection. Thank you very much for your time and consideration. I look forward to working together toward a resolution. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11605788

Something malfunctioned with the brake system of the car. The car was in park and off. I exited the car, reached back into the car for an item, and shut the door. I then walked up the driveway and to the front door. At that point, the car began to roll down the driveway. Someone coming down the road in front of the driveway hit it on the front passenger side. The car then continued to roll across the street and part way up the driveway across the street, coming back across the street and then ran into the same car that hit it. I was then able to open the car door and get inside. It was still off and in park. I depressed the brake pedal, started the car, put it into drive, and moved it to the side of the street. After the incident, I tried multiple times to replicate the incident with my car. I tried leaving it in reverse, even though I know it was off and parked. I also tried leaving it in neutral and turning it off, even though I know it was off and in park. Each time, the car began to roll immediately. There was no way that I could have done what I did that day, which was to step out, reach back in to get a bag, shut the door, and then walk away. The car was definitely off and parked when it happened. The vehicle is currently at a Mazda authorized repair shop, and I am awaiting the final word on whether or not they can detect what exactly caused the brakes to malfunction. There were no warning lamps or messages prior to the issue; however, I did notice that something seemed to be going on with the screen of the car prior to the incident. though the clock was set to the correct time, the screens auto-sensor for day and night mode was not functioning correctly. (When it got dark, the screen was still staying lit up brightly. I confirmed that it was on "auto" mode, and it should have dimmed since it was after 9:30pm and dark out, but it didn't. I had to manually switch it to "night" each time it was dark.)

STEERING,SERVICE BRAKES,ENGINE

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11726252

Incident Description: On multiple occasions in January and February 2026, while operating a 2024 Mazda CX-5 in reverse on low-friction surfaces (snow/ice) with a slight incline, the vehicle suffered a total systemic failure. System Failures: While reversing at low speeds, the engine spontaneously shut off without warning. This resulted in an immediate loss of power braking and power steering. Simultaneously, the electronic gear selector (shifter) became non-functional, preventing the vehicle from being placed into 'Park' to stop the uncontrolled rearward slide. Safety Risk: The vehicle became a "runaway" object on several occasions. In one instance, the vehicle narrowly avoided crushing a pedestrian against another parked car, stopping only when the tires reached a different surface texture. In a separate incident, the vehicle slid uncontrolled down a residential hill, striking snowbanks. There is a grave concern regarding whether the Supplemental Restraint System (airbags) remains functional during these engine-stall events, as the dealer could not confirm occupant protection status during a total power loss. Inspection & Reproduction: The vehicle was presented to an authorized Mazda dealer on March 3, 2026. The service department attempted to replicate the issue by reversing over soapy water on a gravel surface. However, due to the high friction of the gravel, they were unable to induce the specific tire-slip/low-traction conditions required to trigger the malfunction. No error codes were found during an informal scan, and no warning lamps or messages appeared prior to or during the failures. Component for Inspection: The vehicle is available for inspection. I believe the failure is rooted in the traction control or engine management software, which appears to improperly shut down the powertrain when it detects specific wheel-spin patterns in reverse, subsequently disabling critical safety systems including brakes and transmission locks.

My car caught on fire on [CCC] and it's completely burned down, a total loss. Following is information about my car and some summary of what happened. Car information: - Mazda CX5 2024 - Bought in June or July 2024(car payment only made 22 times so far) - Odometer 24,445mi(as of 05/15/2026) What happened: - In [XXX] Friday afternoon, I was in Bellevue and noticed an engine overheat warning. So, I stopped immediately to cool down and called Mazda Dealership(Lee Johnson Mazda) to schedule a service. - Then after 20-30 minutes, I started the car again and the warning was gone. So I drove slowly and got on [XXX] to head home in Kirkland. I was already driving slowly. - After a few minutes, overheat warning as well as engine warning started firing and noticed that smoke coming out of bonnet. - So, I pulled over on the shoulder near [XXX] and called roadside assistance. But even before I get connected to the roadside service, engine caught on fire and I had to call 911. - The car is completely burned down and it's a total loss. - State patrol wrote me a "Exchange of Information" and "Tow/Impound and Inventory Record". Why I am submitting this form: - This car is less than 2years old and has been seeing Mazda service regularly. The odometer checked by dealership on 05/15 is 24,445mi which is slightly more than 24,000 which is stated in the Lemon LAW. But still, the car is pretty brand new and around 24,000 ish. So, I'm wondering if this still qualifies for Lemon Law. - Even before this happened, I had a service history for engine warning and get the engine checked on 05/15/2026 which is exactly a week ago before the accident. Also, they said the engine thermostat had an issue and replaced and covered by Mazda's warranty. - Overheat and engine warning was on and off. As soon as I pull over, it caught on fire. - This happened on [XXX]. It was very dangerous and lucky that we pulled over on right before it caught on fire INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMA

Brake system failure

VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11569176

I have a 2024 Mazda CX-5 but this is in any model and year that has the Adaptive Radar Cruise Control. When you set the distance you want to follow the vehicle in front of you, you will go the speed you are going until you come to the space you have set for the vehicle in front of you. Your vehicle will slow to match that vehicles speed. If that vehicle slows down, you will slow down. What it doesn't do it turn on your brake lights so the vehicle behind you knows that your vehicle is slowing down. Sometimes if the vehicle in front of you slows down fast, so does your vehicle. You have no control how fast it does that, it just does it. Now if the vehicle behind you doesn't realize that your vehicle has done this, well it has been very close two times so far. I do not want my new car rear ended. I did not know it did this with the brake lights and Mazda doesn't tell you that you have no brake lights when this happens. Why would they have a setup like this. It is like brake checking the vehicle behind you. I do not use my cruise control for that reason. It would be nice to be able to use it on a new car but I do not want to get hurt or get someone else hurt by doing so. I did contact Mazda and they know it does that. This is what they told me "Case Number [XXX] Hello. My name is Dominique. I am sorry that you have had some very close calls while slowing down while vehicles were behind you. As for the question you had asked about cruise control activating the brakes when your vehicle starts to slow down, it does not activate the brake lights. It is just like when you are driving without cruise control and you go to take your foot off of the gas pedal instead of using the brake to slow down, it would not activate brake lights either unless you actually mash the brakes, the same with cruise control. Thank you for being a part of the Mazda family. " Isn't that great. I am now a safely hazard or at least my vehicle is, and they are all like that. Who knew. Not me. INFORMATI
