ENGINE

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11564571

Beginning in October of 2023, I was hearing an engine knock/ticking. Acura tech support at the local dealership said my rear cylinder head needed to be replaced. I asked Acura how, since my car is only a few years old, could this cylinder head already be bad. They responded that these vehicles are mass produced by the hundreds of thousands and sometimes bad parts or installations slip through. I paid $7,500 for the new cylinder head, but now I see the new recall about engine crank shaft and the connecting rod bearing can wear and cause damage to the engine. It further says if you hear engine knocking, to immediately call Acura.

2 Issues: Transmission shifts hard between 35-50mph and 50-60mph. Dealership accused me of utilizing improper gas which is not factual; this car has only received the recommended premium octane gas since I purchased it as a Certified Pre-Owned vehicle. Recall: vehicle is currently under crank shaft recall. In the original notification, Acura stated parts would be available in April. Per the Acura dealer, Acura told them parts will not be available until October. This timeline is unacceptable for such a serious safety recall (risk of fire/crash). I feel unsafe driving this car and am worried I will not get fair market value with the recall still open.

After driving for an extended timeframe (approximately 3 hours, trip was traveling between Des Moines, Iowa and Kansas City, Missouri) at Highway/interstate speeds, upon slowing down from those speeds, the vehicle goes into limp mode and check engine light begins blinking. After pulling over and turning car off and on again, normal behavior resumes. This has happened both as I arrived in the Kansas City area, and upon my return home to the Des Moines area. Depending on circumstances for when vehicle enters limp mode, there’s a significant risk of causing an accident. Dealer was unable to pull any codes and therefore is unwilling/unable to address the issue.

Engine had a knocking noise, went to dealer and explained the problem. They mentioned there was a recall regarding this issue but when my vehicle's Vin was typed in they said it was not part of the recall. A few days later all the warning lights in the instrument panel came on and soon after the engine went bad and had to replace it. My vehicle had approximately 45k miles when this happened. I was completely shocked, angry and disappointed with Honda Motors when they didn't want to take responsibility. It's obvious this was a manufacturing problem with these engines and they should reimburse vehicle owners that had issues with these vehicles. Thank you for your attention!

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11481550

The auto stop feature is enabled by default when the car is started. When stopped at a traffic light or in a traffic jam, the auto stop turns off the engine and is supposed to restart when the drivers foot is off the brake. The engine unexpectedly will not restart, and the car must be put in park, turned off, and restarted. This is dangerous in heavy traffic. The dealer replaced the battery as an attempted repair. The problem reoccurred. The dealer then updated the software, which also did not solve the problem. The dealer advised that Honda is aware of the problem and currently has no solution. I was advised to manually turn off the auto stop feature until a solution can be found.

I was driving my car with my infant daughter in the backseat. We were stopped at a red light in the middle of a busy intersection. The car was in idle mode. When I went to turn left the car shifted into neutral. I could not go into drive. I had to turn the car off and turn it back on again to get it to move out of the intersection. We almost got rear ended. When my husband brought the car in the shop they ran diagnostic testing and said nothing was wrong. Only that the problem may be caused by a low battery. They replaced the battery and let my husband drive home. I want to know why there was no low battery warning light? Also why do I have to almost have a collision to find out the battery doesn’t work?

The windshield camera suddenly failed one day while driving and it ended up disabling all the driver assist systems including the emergency braking, lane keep assist, forward collision, auto high beam, cruise control etc. Over the next few weeks the system kept randomly working and failing during driving every few hours or minutes. The discussion forums are filled with people complaining about the same issue with ALL Honda and Acura vehicles starting 2013 where they camera suddenly fails and then start working intermittently until it eventually fails. It seems Honda is using the same defective camera across all it's Honda and Acura models which is a big safety hazard and they refuse to acknowledge the issue.

Rear tail gate lift motors are overheating and won't open rear hatch. The motors will lift the gait for about 5 or 6 openings, then the gate will not lift at all. The vehicle has to sit overnight for the system to "cool down". Once it cools down, the tailgate will operate the next date, but will operate only once. The car has to once again, sit overnight, in order to lift the tailgate 1 time. If the motors of the gate are overheating, this is a safety potential fire issue. At this time I am being told the motors are on back order, and I have no manual override in this vehicle to lift the tailgate hatch. If the same motors are being put back in the vehicle, without any modifications, this will not suffice.

I was driving at about 65mph on the NJ Turnpike at 7:45pm on 8/28/25 when suddenly the accelerator pedal was non-responsive over a period of about 3-5 seconds. The RPMs dropped (engine did not stall) and the vehicle speed began to diminish. No warning lights showed and the dashboard remained lit as usual. The road was wet from a recent downpour. This was a very scary moment as my vehicle was surrounded by trucks. I further depressed the accelerator pedal and was able to regain control of the vehicle. This is the one and only time this happened with this vehicle as far as I am aware. I have scheduled to bring it to Acura service next week regarding this issue. A Google search has revealed several class action lawsuits regarding this issue.

Driving down 45 at 70mph. Attempted to accelerate and change lanes. Car lost engine power and was almost rear ended before power was restored. Have been waiting almost a year for recall to be complete. Grubbs Acura called is the other day to have service performed. Made the aware of the symptoms. They tried to have us pay for a diagnostic. Informed them that the symptoms were that of the recall. We believe. The engine may have damage and should be further inspected and or replaced. The informed is that they replaced the connecting rod through the oil pain and did not touch the timing belts. However it’s time for timing belts and other maintenance to occur in the next 5k miles. Feel like the job was rushed and it is a safety hazard.

I pulled up to a stop light at a busy intersection/highway exit and, after depressing the brake, the vehicle abruptly turned off and shifted to neutral. The brake became very difficult to depress and the parking brake enabled on its own. The entire dashboard lit up and the car would not shift or respond to any input. The vehicle would not turn off, turn back on, or shift for approximately 15 minutes. After this, I tried to turn the car off and it finally did, I started it again and was able to pull off the roadway. We just recently (within the past 2 weeks) had a recall done to update the auto-stop system. This occurred 1x prior to the recall and now this has happened again. My young children were in the car both times and this put us all in danger.

Other cars exiting the highway at a high rate of speed had to go around my vehicle to continue. This is the second occurrence like this and previous “repairs” was the dealership just replacing the battery. This is the second time my family has been put at risk. This vehicle comes with Idle engine feature. when the brake pedal is pressed, the engine turns off (supposedly to save gas). The car should proceed when the gas pedal is pressed. So at any time, in any location, after the car is stopped at a stop sign or red light, the car does not always turn back on. I just sit in the vehicle, unable to proceed. I then have to restart the car in order to drive forward again. This is extremely unsafe, as I could be stalled in an unsafe location. this vehicle had a recall due to this faulty issue, and the sensor was replaced by the Acura dealer, however the problem remains. I feel unsafe driving this vehicle. Apparently many Acura owners have the same problem. This problem could result in death. It is extremely unsafe to have a car stall.

Under normal driving conditions warning lights for "electric parking brake" and "emissions system problem" came on the dashboard and the 'brake' light began flashing. No change in performance. Auto parts store diagnostic indicated EVAP valve as the possible problem code, along with two other unrelated codes - none connected to brakes or emissions. The 12-volt battery tested strong. Drove another 100 miles with lights still on, but the 'brake' light would turn off after about 5 mins of highway driving/no braking. The next day the warning lights continued. After normal city driving I shut the car off, waited 30 seconds, pushed the start button twice (accessory mode) and held the brake pedal down while pulling up on the brake release button for 10 seconds. Started the car without any error messages.

On 04/03/2026, 6:04 PM, Acura MDX 2019, Base, SH-AWD, at 105,254 miles on the odometer, posted U3000-49 control module internal electronic failure. The fault appears to be at the 36160-TYS-A02 camera assembly monocular results in a series of cascading errors on the dash with indicators on for LKAS (Lane Keeping Assist), ACC (Adaptive Cruise Control), CMBS (Collision Mitigation Brake System), brake and transmission warning lights that blink continuously. The sudden lost of safety system especially at highway speeds places extreme risk to occupants of vehicle and other drivers sharing the roadway. The current TSB 23-008 does not repair fault, origin of fault not software, but requires replacement of front camera assembly (36160-TYS-A02) and recalibration at owner expense. There appears to be a lot of 36160-TYS-A02 camera monocular that either have insufficient solder, cold solder, bad crimps or otherwise that over time results in failure.

In March, I made an appointment at Lehigh Valley Acura in Emmaus, PA 18049, to address NHTSA Recall Number 23V-751 on my 2019 Acura MDX. When I got to the appointment on [XXX], the service technician told me that American Honda will only send parts for recall-related repairs for cases where the "vehicle is showing evidence of the connecting rod bearing defect." Since the evidence of the defect is that the car could "stall while driving," I believe this sounds like a defect that should be addressed immediately. I called American Honda on [XXX], and was told that they don't have sufficient parts to fix all the vehicles covered by the recall. The person I spoke to said that we would get a letter in the mail when it is our turn. I believe this is negligence on the part of American Honda and Lehigh Valley Acura. How do I get this defect addressed immediately? INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I was stopped at a light and as the light changed to green I began to accelerate to enter the on ramp to 75N. While driving onto the on ramp many warning lights came on as my car shifted into Neutral on its own. I couldn’t accelerate, switch it back to drive.or depress the break as it was locked. The car stopped and would not move on the entrance ramp to a busy highway. I called 911 because I was almost hit multiple times as I could not move the car. FDOT came to assist and saw that I could not depress the break, remove the emergency break, or get the car to move. After a few minutes we turned the car off and it started back up with no problem. The vehicle had to be towed to the dealer on 3/30/2022. Upon searching on the internet, I saw that this is not a new issue. I could have been killed yesterday while on the phone with 911 on the on ramp to the highway with vehicles approaching at a very high rate of speed and having to slam on their breaks and swerve off the road to avoid hitting me.

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11492321

When stopped at a light or in traffic the vehicle engages the auto idle feature. (turns off the engine to save gas). Upon releasing the brake, the car should re-start, instead the car does not start. The car jumps out of the drive gear into neutral on the transmission. The electrical system goes crazy and does not allow you to either turn off the car's electrical system or re-restart the car using the car's on/off button. This has happened to me at least 5-10 times over the last year. Each time the car becomes immobile in the middle of the road or highway for up to 2 minutes, until the electrical system resets and allows you to turn off all the power and re-start the car. It's a very scary and dangerous situation, when my vehicle is stranded in the middle of a highway or a road for up to 2 minutes, as traffic tries to avoid crashing into my vehicle. Searching online, I was able to see that this same condition has affected many MDX owners but Honda does not seem to have recall or fix for this issue.

Dec. 16, 2025, took Acura into the dealership because Check Engine light kept coming on. They did a full inspection and said I needed to have the purge valve replaced and that it was nearing time to have other services: transmission service, front transfer case, rear differential, brake fluid, timing belt, spark plugs, fuel service. Took the diagnostic to my brother who is a mechanic and he started the work starting with the purge valve. He had already done spark plugs with oil change in Oct. 2026. Fast forward to Apr. 2026 my car starts making a weird knocking sound upon accelerating. My brother was in hospital at this time so I didn't want to stress him so I just stopped driving it. I had the other stuff done at shop he recommended but the knocking sound didn't stop. Brother told me to get it diagnosed again. Now Sept. 2026, they said I need a whole new engine, and that Acura would not cover it! What!? They felt so badly they didn't even charge me for the diagnostic. I took it to two other shops just to see if there was anything else that could be done but they all told me the same thing. It's a 2019 how on earth could it need a new engine. I've gotten ALL the work done that is recommended. I didn't even drive it when it started making that noise. My husband and I have been sharing 1 vehicle! I have not missed an oil change and had all recalls repaired there but the dealership told me since I didn't do the services in the dealership at a premium that Acura would not extend me a Goodwill to replace their faulty engine. Why would I do the work at the dealer when my brother is a mechanic and owns his own shop? I believe my engine is suffering the same issues that are causing millions of Acura owners the same dangerous recall. The last place I took the car was the Acura/Honda Specialists shop in the city where I live and this guy told me everything that was serviced looked good but the car needs a new engine and there's even metal in the oil but my VIN doesn't count!

While making a very normal commute, my 2019 Acura MDX began running very roughly, slowing rather abruptly. The check engine light began flashing, and several warning messages were displayed on the message center of the vehicle display unit. The vehicle would not accelerate, and would not attain a reasonable speed while in traffic. With the hazard lights on, I was able to move to the side of the road, but it was very difficult as I was unable to exceed 15-20 mph. The vehicle was taken to the dealership for repair. The dealer investigated and found the timing belt had "jumped time" causing the engine management system to set multiple misfire codes, and was described as the cause of the failure. I am aware certain 2018-2019 Acura MDX models were recalled for timing belt issues, but this particular vehicle was determined to be outside of the group of vehicles involved in the current recall. My MDX had 52,600 miles at the time of the incident. It has not had any modifications or repairs to the engine prior to this incident. Acura recommends replacing the timing best at 100,000 miles.

Camera is failing - reportedly due to a bad internal ground-wire. Acura is charging $900 for the part and $1600 for the installation and calibration of a new unit !! Also - the unit is a black color, and located right behind the windshield, so bakes itself to death. Acura should be using white colors inside the space, limit the un-used volume, and recalling the units (or at least providing a low-cost unit) to allow simple replacement and improve safety. Such an important AND EXPENSIVE part should not be failing so often. The MXers.org web site has hundreds of posts about this item. ALSO - the parts should be pre-calibrated! Acura wants several hundred $$ to calibrate it and 4 days to do it. NHTSA should NOT be requiring ADAS technology if it is so fragile and expensive to fix/replace, as in the long-run owners will not maintain it (e..g $2,400 repair bills for a small camera part nad 5 minute change). My car has stalled while driving on the highway multiple times now. The first time it happened, it lost power, stopped accelerating, and cycled through multiple warning lights, causing a significant safety risk. I pulled over and it restarted within minutes. I took it to a repair shop and they could not find a problem with it, even with the codes provided. Nothing pointed to a solution. It happened two other times while I was driving my child on a highway where it lost acceleration and then kicked back in. Again, this post, a significant safety risk as always in a fast lane on a highway with my child cars behind me had to hit their brakes to avoid me after this incident. Other times it doesn’t lose power, yet it cycles through warning lights randomly. I took it to the dealer instead of a local shop. They texted me about items that should be fixed per an inspection, including struts and a battery replacement. Complete with a large bill just for diagnostics. Yet, nothing was found having to do with the main problem at hand. Losing power while driving. . My battery is less than six months old. I had it tested and it is fine. My vin is not listed in the recall, but, all of the symptoms match the other recalls of the same year make and model.

My 2019 Acura MDX with 100K miles stalled on the road with several alerts on the dashboard and went into Parking mode with no further movement. Had several alerts stating Transmission failure, Blind Spot monitoring failure, Auto break system failure and vehicle stalled on the road. Had similar issues previously where the steering jammed and vehicle was stalled on the signal, took it to Acura dealer and they had to fix some circuit / reset the circuit board. Was ok for few months and last week the car stalled with Parking sign on. Towed the car to the Dealer service center, they diagnosed and identified there is a failure with transmission. They quoted around 10000 dollars for replace new and 7000 for used transmission. This is huge money, hence called Acura customer care for any review/support but they claimed this issue is not in their current recalls or claims, so cannot support. had to pay out of the pocket. Per google there seemed to be a lawsuit with Transmission failures with Acura 2019 mdx vehicle. Acura customer relations is asking to get into the law suit group and Acura as such cannot help the customers with these problems. Had Lexus for several years with no issues and very upset/annoyed with these type of repairs within 100K / 5year old.

The vehicle was towed to Mack's Automotive but due to the extensive damage, the vehicle was not repaired. The contact was informed by Mack's Automotive that the rod went through the engine causing the oil to leak which caused the engine to catch on fire. The vehicle was towed back to the nephew’s residence and remained unrepaired due to the cost and because the Insurance provider declined to pay to repair the vehicle because it was deemed a mechanical failure. Infotainment system experiences periodic failures causing inability to control climate/temperature controls, communications, navigation, and vehicle settings including safety settings. It occurs when driving in city and on highway regardless of conditions or speed, when stationary, can at times be corrected by pulling off the road and restarting the car, but sometimes it just takes time to resolve. Acura stated this is caused by the connection to the cell phone, but occurs even when no phone is connected to or in the car. This issue is a significant distraction to the drivers attention, and can for exposure to extreme to outside temperatures below freezing and above 95F in high humidity mo summer heat. Multiple attempts to repair the vehicle have failed, arbitration with ncds ruled in my favor for Acura to repair the vehicle which they cannot do, it is a known issue with Acura vehicles and they continue to sell, and there are multiple current class action lawsuits against Acura and their parent Honda motor company for the identical issue with different models from the same Model year. My attemepts to trade and sell the vehical have been thwarted by occurances of critical failure of these electronic/computer based systems. I am currently arbitrating Acura (non-legal) for the resolution of these unsafe driving experience.

The subject 2019 Acura MDX model includes the tech package which adds several safety features that function through a windshield mounted camara/radar. The system has malfunctioned and none of the safety features are working. The dash displays the following messages: Collision Mitigation System Problem Adaptive Cruise Control Problem Lane Keeping Asist Problem Road Departure Mitigation System Problem automatic headlight system problem At the bottom of the dash, I see Brake System, Crash Warning and Lane Departure lights on as well. Acura is charging $2,319 to replace this unit in addition to a $270 diagnostic fee. This issue is widely reported as a significant issue in several model years for the MDX and should be part of a safety recall. The Acura technician acknowledged this issue is common. I should add this is the second major electronic issue with this MDX within the last several months. The prior issue was multiple system failure warnings that Acura determined were caused by a faulty wiring harness at a $1,604 cost, plus a $255 diagnostic fee. Please note I am the original owner of the vehicle and it has never been in a collision. Additionally, the system has recently mistakenly activated the brakes at very low speed and warned against collisions with other vehicles traveling in the opposite direction while on a curve.

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11736180

I am reporting a recurring safety issue following the Acura rod bearing recall repair on my 2019 Acura MDX SH-AWD. The original recall resulted in catastrophic engine failure and Acura replaced the long block assembly. Since that repair, the vehicle has repeatedly failed and returned to the dealership multiple times for the same issue pattern. Symptoms include: * “Emissions System Problem” warning * SH-AWD, brake, and traction control warnings * strong raw gasoline odor * vehicle becoming undriveable/no-start Timeline: * Recall repair completed with long block replacement * Vehicle failed again the next day * Dealer repaired vehicle * Vehicle failed again about one month later * Dealer repaired vehicle again * Vehicle failed a third time about one week later during normal commuting The recurring failures are accompanied by a strong gasoline smell, creating concern about fire risk and vehicle safety. I regularly transport children in this vehicle and no longer have confidence in the safety or reliability of the recall repair. The dealership has continued repairing the vehicle and providing loaners, indicating the issue is related to the original recall repair, but the underlying problem has still not been resolved despite multiple repair attempts. I am submitting this report because the recall repair has not resulted in safe and reliable vehicle operation.

POWER TRAIN,ENGINE,FUEL/PROPULSION SYSTEM

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11442525

I think this recall needs to be expanded: Honda and Acura have recalled 2019 and 2020 vehicles including the CR-V and Accord for fuel pumps which can malfunction and cause the engine to lose power. My wife recently had her 2019 MDX stall on her in an intersection and she had to turn off the car and turn it back on so it would work. It sounds exactly like the below: “While driving, the vehicle shifts into Neutral, the transmission indicator comes and DTC P0657 (actuator supply voltage circuit/open) is stored. The driver will not be able to select any other gear until the vehicle is turned off and restarted. Update the PGM-FI [programmed fuel injection] software and the TCM software, clear any DTCs [diagnostic trouble codes], and do the PCM [powertrain control module] idle learn procedure.” - TSB 15-034 Additionally, May 23, 2021 we had an experience where the same MDX decelerated as we merged into traffic at I-30 and I-820 in Ft. Worth. I let off the accelerator going 60-70 MPH and then stepped on the accelerator to complete my merge and there was no power. Speed dropped to around 20 almost instantly. I had to completely let off the accelerator and depress it again to get going. Very scary situation considering how fast people drive on Texas freeways. These are two big issues Honda/Acura needs to address ASAP as both put consumers at risk of injury and possibly death.

FUEL/PROPULSION SYSTEM

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11625723

In February 2024, received a recall for fuel pump module, which can result in prevent engine from starting, cause vehicle to lose driver power, or stall while driving. Can also result in failing emissions inspection. Acura reports that repair is not available until Fall 2024. In October 2024, while at a stoplight, auto idle-stop was engaged, and engine stalled and would not turn on. Vehicle needed to be turned off and then engine started at that time. A previous recall that affected auto idle stop was addressed in May 2023 where engine does not auto restart after entering auto idle stop. In April 2023 Acura provided a software update, however in October 2024 the engine stalled as mentioned above. Additionally, a local emissions test was performed and failed around April 2022, and passed a second time when performed a few weeks later. Even after experiencing symptoms of recall 23V-858, recall has not been able to be addressed. This vehicle was brought into Acura in November 2024 for an another recall (23V-751) and I was told that current unaddressed recall may be available within the following month but could not provide any further assistance. Engine stalling at a stoplight puts my family at risk, as well as others. If engine is stalling at a stoplight, engine may stall at other times, including while driving, increasing risk of accident. Recall has been undressed for almost 10 months.

EXTERIOR LIGHTING

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11645250

The headlight assemblies to my vehicle have become defective. The headlight assembly, both right and left, has moisture intrusion within the headlight assembly, creating condensation build-up inside the housing. This issue also caused the headlights to become white and hazy in appearance. I was advised by the dealer that this would lead to failure with the lighting and would have to be replaced. I purchased a certified used vehicle with Tustin Acura in August of 2022 and also purchased an extended warranty for piece of mind. In the 2 1/2 years that I have owned the vehicle, I have added about 25k miles and have always garaged the vehicle. When the issue was reviewed by the dealer, they refused to cover the problem under warranty and requested that I reach out to American Honda for further assistance. I did that but was denied and told the issue was caused by "outside influence," but provided no explanation as to what this meant. To me it felt like I was being blamed for clearly a defective issue. A Google search of this issues revealed to me that this was a common issue with 2019 Acura MDX vehicles. To replace the headlights would be a cost to me of nearly $3,000. it seems almost ridiculous to pay for clearly defective equipment. I fear that my inability to afford this great expense with lead to headlight failure that could result in a collision if this failure were to occur at night.

I was merging onto the interstate from another interstate when my car suddenly became limp and would not accelerate. I had zero warning there was anything wrong with my car, only 67,150 miles. I had to coast to the side of the road and it wasn't driveable. I was forced to have it towed to Acura to find out it's a rocker arm oil pressure switch that apparently goes out after 60,000-100,000 miles. Without warning, this is a huge safety issue. There should be a backup switch and notification to get it to a dealer. I had 3 kids in the car, traveling 65 when it suddenly decided to stop accelerating. ZERO warning. It was terrifying. I could have been hit by another car on the interstate, with 3 kids in the car, zero warning to cars behind me that my speed went from 65 to almost nothing instantly. I've always owned Honda cars and this is the first vehicle I've ever had this happen in. Dealership confirmed it was a faulty switch, nothing to do with low oil, dirty oil and I have that in writing. Acrua/Honda should figure out how to alert users that there is an issue before there is an issue. Or put this on the maintenance schedule because I'm terrified to drive this car now. Or how will I know if it's ready to do it again. Again, some type of backup switch that works for a bit until you can get it to dealer. I had to be towed to dealer. Kids had to be picked up. All things that could have been avoided if Honda did a better job.

The contact owns a 2019 Acura MDX. The contact stated that he recurringly experienced failures with the instrument panel brightness being dimmed and then not displaying properly as needed. The contact turned off and restarted the vehicle and the instrument panel operated as designed. No warning lights were illuminated. The vehicle was taken twice to a local dealer; however, the failure was not duplicated, and the vehicle was not repaired. On another occasion while driving and using the GPS, the displayed failed operate as needed. After turning off and restarting the vehicle, the instrument panel operated as needed. The vehicle was then taken back to the local dealer however, the mechanic was unable to duplicate the failure. The contact stated that the vehicle was however, repaired under NHTSA Campaign Number: 21V215000 (Fuel System, Gasoline). After the repair, the contact stated that the vehicle fuel tank was at half full however, the fuel gauge indicated that the tank was empty. The contact stated that the instrument panel then became inoperable. The failure persisted and the contact would turn off and restart the vehicle for the instrument panel to operate as needed; however, the speedometer would remain inoperable. The vehicle was not repaired. The contact sought legal advice and his attorney advised him to file a complaint with the NHTSA Hotline. The manufacturer was not made aware of the failure. Powertrain / transmission failure

POWER TRAIN

69 months

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11668367

At only 44,318 miles, my 2019 Acura MDX began showing clear transmission problems in the lower gears — including delayed acceleration, gear slipping, and jerky movement when starting or turning at low speed. These symptoms posed an immediate safety risk, especially when merging into traffic or making turns across oncoming vehicles. I first brought the car to Oakland Acura, which incorrectly diagnosed it as a transfer case issue. Their misdiagnosis delayed the repair process and added unnecessary cost and time. I then had the vehicle evaluated at Acura Concord, where it was correctly diagnosed as transmission failure in the lower gears. An independent, certified mechanic also confirmed the same diagnosis. Despite multiple confirmations of premature transmission failure — and a full maintenance history — American Honda offered only a $900 discount on a $9,600 repair, citing the vehicle being just outside the time warranty (but with low mileage well under 50K). I believe this is a manufacturer defect, not a wear-and-tear issue. Safety Concern: This transmission failure is not just inconvenient — it is dangerous. The vehicle hesitates when accelerating from stops or low speeds, which can lead to accidents at intersections, highway merges, or left turns. The misdiagnosis further delayed identifying and addressing this issue. I am filing this complaint to urge NHTSA to investigate whether this is part of a broader pattern with Acura MDX transmissions, and to help prevent similar risks for other drivers and families.

UNKNOWN OR OTHER,LANE DEPARTURE,FORWARD COLLISION AVOIDANCE

Vehicle Make/Model/Year: 2019 Acura MDX Component Affected: Forward Collision Avoidance / Lane Departure / ADAS (Front Camera System) Summary of Issue: I am filing this complaint regarding a recurring and dangerous issue involving the front windshield-mounted camera on our 2019 Acura MDX. After purchasing this vehicle and driving approximately 5500 miles in Las Vegas, multiple critical safety warnings began appearing on the dashboard, including: •“Collision Mitigation System Problem” •“Road Departure Mitigation System Problem” •“Lane Keeping Assist Problem” •“Adaptive Cruise Control Problem” These warnings occur continuosly. The warning lights won’t turn off, resulting in the complete shutdown of essential Advanced Driver Assistance Systems (ADAS). Safety Concerns: This camera-related failure disables major safety features designed to prevent accidents. The driver is left without automatic emergency braking, lane keeping, or adaptive cruise control. This is especially dangerous at highway speeds. Acura has not issued a recall, despite the widespread nature of this problem and documented cases in other hot-weather regions. Service Experience: We purchased the vehicle used from a Cadillac dealership. The issue began approximately 3 months after purchase. A mechanic confirmed that the front camera system is likely malfunctioning due to heat exposure—a known problem with this model. Why I’m Reporting: I believe this is a safety defect that should prompt investigation. Acura appears to have issued Technical Service Bulletins (TSBs) but has not taken action to proactively notify owners or issue a recall. This defect compromises driver and passenger safety and puts others at risk.

ELECTRICAL SYSTEM

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11349561

While driving at highway speed, there was single "pop" sound that seemed to come from inside the dashboard between the on demand multifunction display (odmd) and the instrument panel. The navi screen then became solid black. After a very short time and while still at highway speed, the navi screen briefly showed the view from the backup camera. After a few seconds both the navi screen and the odmd screen became solid black and remained that way. Then, within seconds, light colored, grayish smoke began flowing out into the vehicle from the cd slot on the odmd. Immediately, there was an odor in the car like that from burning plastic or electrical insulation. As soon as possible the car was pulled off the highway and the engine was turned off. The smoke continued to come out of the cd slot until the engine was turned off, then the smoke stopped coming out. After several minutes the engine was restarted and the car was driven to a safer location.there was no smoke coming from the cd slot after the car was restarted and while driving the car to the dealership for service the next day. An odor like that from an electrical fire remained noticeable in the vehicle. Prior to the event, the driver observed no vehicle warning indications, and both the navi and the odmd appeared to be functioning normally. The cd player was not being used. The dealership service manager advised me that they consulted with Acura engineering personnel, inspected and tested wiring inside the dashboard and have isolated the cause of the problem to "a short in the radio." The dealer says that the odmd and the hard disk drive have been sent to alpine to determine the cause of failure at alpine's request. I have asked the dealership to confirm that the repairs to the vehicle will ensure that this kind of event will not recur.

ELECTRICAL SYSTEM,UNKNOWN OR OTHER,FUEL/PROPULSION SYSTEM

25 months

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11473342

I have had problems with the car stalling or shifting to neutral from drive after the auto stop feature is engaged. This typically happens when the engine tries to restart after having shutoff at the stop. It is difficult to replicate the problem and the dealer can't seem to replicated the problem but it has happened to me many times, particularly in hot weather or after long drives. Typically it happens after a long drive or a lot of around town driving (maybe when the engine is hot?). The dealer does not believe it is related to the fuel pump replacement but that is when it seemed to have started happening but this is entirely speculation. Very frustrating as I have been to the dealer 3 times with the problem and they cannot "replicate" it. They supposedly also replaced the battery and updated the software in the car but that has not solved it. I am growing concerned because it is very dangerous, particularly when stopped at a busy intersection. The last stall happened and it took a minute or so to get the car running again at a busy and large intersection. I am also worried about the happening when on an incline. It doesn't happen all of the time, sometimes there is just a hesitation but the car starts and lurches forward. I have a 2019 with about 35K miles on it and have used primarily for long distance trips and not much around town driving. I have attached the dealer work orders for the three visits but stopped bringing the car in because it has been a waste of time as they are unable to find the issue. Also, I have reported the issue twice to American Honda and they have sent me back to the dealer with no results. This is a dangerous situation and I noticed that there is a related recall for the Acura TLX model with the same engine. This problem is ongoing and has happened to me twice in the past 2 days if I forget to disable the auto shutoff feature. Thanks.

Since the purchase of my Acura MDX 2019, the following problems have been reported to the dealership and to Honda Manufacturing Inc. without any resolution. The dealer shop has replaced recall parts, but the problems persist and have not gone away. The vehicle stalls and stops working while driving. The engine makes knocking/rattling noises upon becoming disabled while driving. The engine would randomly not start when turning it on. When car becomes disabled the dashboard lights up warning lights displaying messages intermittently of the following problems: Anti-Lock Brake System Problem (ABS), Vehicle Stability Assist (VSA), Hill Start Assist, Trailer Stability Assist, Collision Mitigation System, Road Departure Mitigation System, Adaptive Cruise Control, Lane Keeping Assist, Brake System, Brake Hold System, Electric Parking Brake, SH-AWD, Keyless Start System and Power Steering System (EPS) Problems, while displaying ACC and LWKS codes. Additionally, the vehicle runs improperly and shuts down completely making the window, doors, etc, inoperable while passengers are inside locking me in and my entire family for long periods of time. A few times has left me either myself or with my children and dog inside. The only place where I take my car for services is where I purchased it (Chevy Chase Acura). The issues noted are the same reported by the recalls I have received, and I have not taken it somewhere else because of that reason. Nobody can fix them as the problems are manufacturing issues that even Acura/Honda cannot repair. This safety defect can kill or seriously injure me, my passengers, and other drivers on the road. As noted in one of the many recalls the fuel pump defect can cause the vehicle to stop in the highway and could prevent an engine from starting or cause a vehicle to stall while being driven, it potentially increases the risk of a fire, crash, or injury. My car continues to have these issues and nobody is helping me. Please help!

FUEL/PROPULSION SYSTEM

83 months

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11745700

On [XXX], while traveling at highway speeds on [XXX], the vehicle suffered a sudden, complete loss of engine propulsion. The vehicle rapidly decelerated, refused to respond to accelerator input, and forced itself into a dangerous "Limp Mode" in live traffic, creating an immediate highway safety hazard. Multiple warning messages illuminated on the dashboard. The vehicle was towed to Mile High Acura under Repair Order #[XXX]. Diagnostic testing pulled stored Code P0087 for Low Fuel Pressure at the Fuel Rail. An inline fuel pressure gauge confirmed that the low-side fuel pressure was rapidly fluctuating between 55 and 64 PSI at idle. The dealership escalated the data to the Acura Corporate Tech Line (Ref #[XXX]), which reviewed the vehicle's onboard snapshot data and officially determined a terminal defect in the low-pressure fuel pump assembly, mandating the replacement of the low-side pump and both fuel relays. Dealership service records confirm that this vehicle previously had a low-pressure fuel pump recall performed in 2021 under NHTSA Campaign Number 21V-215 (Acura Code CAI). The sudden failure of this replacement component matches the exact failure conditions detailed in the subsequent NHTSA Campaign Number 23V-858 (Acura Codes KGC/KGD). In that expanded campaign, American Honda Motor Co. openly admitted to federal regulators that the early replacement "service parts" distributed and installed during the 21V-215 window were manufactured with the exact same defective, swelling density impellers prone to subsequent deformation and complete failure. Despite their own corporate Tech Line explicitly confirming that the root cause of this dangerous highway power loss was the low-pressure fuel pump—the exact component of the 21V-215 and 23V-858 safety recall campaigns—Acura Client Relations denied 100% safety remedy coverage. Acura refused to provide the technical basis for their denial in writing. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA),
