STRUCTURE,VEHICLE SPEED CONTROL

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11615318

I am writing to report two concerning safety issues with my vehicle: Repeated Door Clicking During Car Wash: On multiple occasions while driving through an automatic car wash, I have noticed that the car doors repeatedly click, as if attempting to unlock or open. This erratic behavior occurs despite no input from the driver or passengers, raising concerns about the vehicle’s electrical system and its reliability under normal operating conditions. Speedometer Malfunction: More critically, the instrument panel, including the speedometer, has gone completely black while driving. The panel remains non-functional for 5-10 minutes before rebooting on its own, leaving me unable to see my speed or other critical driving information during this period. This issue poses a significant risk to my safety as well as the safety of others on the road. Both issues are unpredictable and have occurred multiple times, making the vehicle unsafe under certain conditions. I kindly request your assistance in investigating these problems to ensure that no further safety hazards arise.

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11603223

The contact owns a 2022 Volkswagen ID.4. The contact stated that there was a message to update the software displayed. The contact stated that while driving at slow speeds, the vehicle came to a complete stop. While driving at various speeds, the camera and the infotainment screen were inoperable. The vehicle failed to start as needed. While the vehicle was idling, all the doors were locked, with the contact holding a key fob. The vehicle was taken to the local dealer, where it was diagnosed and determined that the sensor needed to be replaced. The vehicle was repaired, but the failure recurred. The contact received notification of NHTSA Campaign Number: 24V344000 (Back Over Prevention, Equipment, Electric System); however, the part to do the recall repair was not yet available. The local dealer was contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the issue. The contact had not experienced a failure. The failure mileage was approximately 5,000. VIN tool confirms parts not available.

ELECTRICAL SYSTEM,VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE

42 months

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11705922

My 2022 VW ID.4 Pro S has experienced a total failure of the GPS and telematics system (J949 module), causing a dangerous 'GPS Drift' where the vehicle reports its location miles away from its actual position. This creates two severe safety hazards: Automated System Malfunction: The Travel Assist and Dynamic Road Sign systems are receiving conflicting data. The camera correctly identifies a speed limit (e.g., 40 mph), but the faulty GPS overrides it with incorrect map data (e.g., 25 mph). This causes the vehicle to unexpectedly and dangerously decelerate while in active traffic. Emergency Service Failure: The eCall (Emergency Call) system is reporting the wrong location. In the event of a serious crash, emergency responders would be directed to a location miles away from the actual accident site. My dealer provided written documentation (attached) stating there is 'NO FIX' currently for 2021-2023 models, as they are hardware-locked to a failing Verizon-based data protocol that VW has not patched. The manufacturer has acknowledged the defect but offers no remedy for this safety-critical system.

We've taken the ID.4 in to the dealership three times for repairs. They have not been able to resolve it and have more or less said that it is a manufacturer defect. * The car does not connect to the internet. * The driver's console continues to go blank, very often loses all safety features, and generally acts oddly. We've realized that this mostly happens when the car travels over slight bumps or vibrations, which is basically every time we drive it. Warning signs on the driver's console will show loss of main safety features like Rear Traffic Alerts, Lane Assist, and Cruise Control. * The main driver console often goes blank. It recently blanked out for twenty to thirty minutes while driving on the freeway, making it impossible to know how fast I was going. This also happened in a school zone where children were present. * The back over camera often goes blank. There are children in our neighborhood and this makes backing out of our driveway unsafe. I've supplied Volkswagen with multiple videos of these occurrences, including to vwcustomercare@vw.com and safety@vw.com, and they have not resolved the issue.

FUEL/PROPULSION SYSTEM,FORWARD COLLISION AVOIDANCE

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11565458

While driving to work at 6:45AM, on a secondary road with the vehicle in "B" mode and the main cruise control switch active, but not on, I approached an intersection and was attempting to use the vehicle's regenerative braking. My right foot was off the gas and gently resting on the brake peddle. This is when I felt the car suddenly accelerate. I immediately pressed the break peddle which slowed the car appropriately, but not to a complete stop. I released the break at which point the car re-accelerated. I immediately reapplied the break and came to a complete stop. There was a car in front of me and if I had not reacted so quickly it's possible I would have run into that vehicle. After these two episodes I immediately pulled off the road into a parking lot and got out of the car to do a visual inspection. After seeing nothing wrong I proceeded to drive to work without incident making sure the cruise control main switch was deactivated. The car is waiting inspection by the local dealership and available for inspection. The problem has never reoccured. No warning lamps nor messages present prior to or during the incident.

Airbag / restraint failure

AIR BAGS

13 months

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11535525

In July 2023, the airbag light came on in the dash of the vehicle. Upon inspection by the VW dealer, Sheppard Motors, the dealership indicated that the airbag module failed. They said the repair part was back ordered and were not sure when it will be available. I asked them does this mean the airbag system will not work in an accident? They said yes. I suggested pulling an airbag module from one of the cars in their inventory and using it to repair the car. They said no. They left us drive the car away with the airbag light still on. I researched on the internet finding a airbag module for the car. I informed them of the research and website. They said they called the other VW dealer but the part was not available even though the part was showing as available on the website. I believe driving a vehicle without a functioning airbag system is very dangerous. I don't understand why the manufacture does not have parts available for a new car. I think the safety board needs to investigate this parts availability problem and airbag module failure. How can the vehicle have such a great safety rating if the airbag system does not work?

When I was driving, I stopped at a traffic light. When the light turned green and I pressed the accelerator, many warning lights turned on and the vehicle did not go forward. After about 15 minutes, I was able to turn off the car and restart it again and move forward. At the next red light, the same problem happened again. After another 15 minutes, I was able to again turn off the car and restart it to get to a parking lot, where it had to be towed to the dealership for service. This was a safety concern because I was stuck in traffic as other cars were in motion around me during morning rush hour. I had to get out of my car to turn it off and back on from my key fob. VW diagnosed the problem as a broken motor. Since my car is electric, it does not have an engine, so the motor is in place of the engine. This happened Sept 6, 2023. VW has still not fixed the car. I tried to upload the "pre-work order" from the service department, which is all I have since the repair invoice is not complete yet, but the upload function on this website is not working for me. I am reporting this in case it is not an isolated incident; if this is not just a lemon of a vehicle, then I am concerned for the safety of other drivers and passengers.

EXTERIOR LIGHTING

25 months

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11605019

During a recent trip to Oregon, my Volkswagen ID.4 experienced a critical failure of both the interior and exterior lighting systems, including the headlights and emergency lights. Initially, upon returning from a walk, I found the lights were on despite the vehicle being locked and not in the "Auto" mode as I had left them. Locking and unlocking the vehicle did not resolve the issue. When I entered the vehicle, a warning light indicating a malfunction in the lighting system appeared on the dashboard, and subsequently, all lights stopped functioning entirely. This malfunction posed a significant safety risk to both myself and other road users, as the failure occurred during a time when proper lighting was essential for visibility and safety, potentially leading to a dangerous situation, especially as it was almost evening. The problem has not been reproduced or confirmed by a dealer or independent service center at this time. The vehicle has not been inspected by the manufacturer, police, insurance representatives, or other relevant parties, although a video recording of the incident is available for review and is attached to the report in the PDF file. Prior to the failure, there were no warning lamps, messages, or other symptoms indicating a potential issue. However, during the incident, a warning light for a lighting system malfunction appeared on the dashboard.

VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE

33 months

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11646763

I was turning into a parking spot in front of the entry doors to a Sheetz convenience store. I was slowing down, and had gotten halfway into the space. All of a sudden, the car surged forward, went over the curb and hit the bollard in front of the parking space. It was hit with such force that the bollard bent and went under our car. My memory is that I was pressing on the brake very hard, once the acceleration started. My husband was in the car with me and can attest to all that I have stated. If that bollard had not been there, and stopped my car, I feel certain that my car would have crashed into the store, seriously injuring (or killing) us and those inside the store. We are planning to report the accident to Volkswagen, and to request that they test the "black box" inside the car, to see if they can find out what went wrong. I have spoken to someone in the Sheetz corporate office , and she told me that they have a video recording of the event. She said she observed my car entering the parking space very slowly, and that the car suddenly accelerated. She is planning to provide that video to State Farm Insurance, our insurance company. Unfortunately, I did not take pictures, myself, of the damage to the front of the car. But we will be requesting that the collision repair shop take pictures before the car gets dismantled for repair. We do not recall hearing any warning signals of the impending crash, and certainly did not experience any emergency braking. The airbags did not deploy.

TIRES

15 months

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11555078

Brand new Volkswagen electric vehicle model ID.4 Pro AWD delivered to us in July 2022 after order was placed in summer 2021 and it was built and shipped from Germany. Tires were manufactured in Poland and came installed with the vehicle. Both front tires (Bridgestone Alenza Sport A/S Size 235/55R19 105T) exhibit rubber peeling where chunks of rubber fall off the tire after only about 1.25 year of driving and approximately 15,000 miles. This presents a safety concern. VW no longer installs these tires as OEM as they switched to Pirelli Scorpion Zero or Kumho Crugen starting with model year 2023. Only first two ID.4 model years of 2021 and 2022 are affected. Note there has already been a recall on Alenza Sport A/S tires but not this exact model per NHTSA Campaign number 22T021000 recalling Bridgestone Alenza Sport A/S tires, size 235/55R20 102V (DOT date codes 2422 through 2722). VW blames Bridgestone as the tire manufacturer and Bridgestone blames VW as they claim they manufactured the tires to Volkswagen specifications. Neither will take full responsibility. The vehicle comes with staggered tires and the back tires are the same but different size (Bridgestone Alenza Sport A/S Size 255/50R19 107T). I am requesting that the NHTSA approve a recall of the OEM tires on VW ID.4 2021 and 2022 model years as described herein. This peeling manufacturer defect is a well documented issue online in different ID.4 owner forums: [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Vehicle Information: Year / Make / Model: 2022 Volkswagen ID.4 VIN: [XXX] Current Mileage: ~58,000 miles Affected Component(s): Electrical System, Power Steering, Structure: Door Assembly (NHTSA Recall 24V-651 / VW Recall 57J9) Incident Description: Immediately following a car wash, the vehicle suffered a catastrophic electrical and CAN bus network failure. A cascade of sensor and system failure warnings illuminated on the dashboard, total loss of power steering occurred, and the vehicle entered a restricted limp mode limiting speed to under 1 mph, rendering the vehicle completely inoperable. This issue is identical to the safety defect described in NHTSA Campaign 24V-651 (VW Recall 57J9), wherein water intrudes past exterior door handle seals and shorts out the vehicle's electronic control network. Door handles became inoperable. Safety Hazard: Sudden, complete loss of power steering and propulsion creates an immediate crash hazard and leaves vehicle occupants stranded in potentially dangerous locations without critical vehicle control functions. Dealer / Manufacturer Refusal to Repair: Upon delivering the vehicle to an authorized Volkswagen dealership, both dealership staff and Volkswagen of America refused to perform the recall repair or waive diagnostic fees. They claimed the recall remedy had already been performed over 12 months prior and argued that any warranty on the recall repair had lapsed. Volkswagen corporate is actively refusing to remedy an unresolved, recurring federal safety defect that renders the vehicle unsafe to operate. POWER TRAIN,ELECTRICAL SYSTEM

11 months

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11521085

I was driving in stop-and-go traffic on 5/3/23 and when I released my foot from the brake to gentle accelerate when traffic ahead of me moved, many error messages flashed on the dashboard and the vehicle did not move. The various errors included "travel assist not available", "forward collision warning not available", "vehicle should NOT be towed", "all wheel drive function is currently limited" and "electrical system warning, please service vehicle". When I pressed the accelerator, the car moved extremely slowly - less than 1mph. I put on my emergency flashers and attempted to move into the right lane and pull over to a safe location (extremely slowly with traffic honking and going around me). I tried turning the car off and turning it back on. Many error messages again, but the car did go a few more feet for me to turn onto a side street and park. At that point the car also started shaking/shuddering. I got out of the vehicle in a safe location and called a tow truck, and the vehicle was taken to the VW dealership. The next day, the service manager told me that they ran the fault codes and started a VW tech ticket and the problem was failure of the rear drive motor (the main motor of the car), which is currently being replaced. I also had a similar incident ~6 months ago where the car suddenly lost power while backing up in a parking garage. After about 10-15 minutes the electronic errors seemed to reset and the car drove normally, but I did take it to the dealership and At that time the control module was replaced. Suddenly losing power while driving is a safety issue because it could easily cause a rear end accident, and it is very difficult to even navigate to a safe location to pull over.

The Marin Volkswagen dealer informs me that the JX4 electric drive motor, control electronics 2 and high voltage control module on my 2023 Volkswagen ID.4 all failed and need to be replaced. I am informed that a Volkswagen field representative went out to the Marin dealership and confirmed the necessity of these repairs. On [XXX], 2024, I was driving in the fast lane of [XXX] at a speed of somewhere between 65 /70 miles per hour when I heard a loud "pop" sound and a warning appeared on my dashboard indicating a system failure and warning me to immediately pull over. I had lost all motor power and had to use whatever remaining momentum I had to immediately get over to the right hand shoulder in rush hour traffic. Mine and the safety of all other drivers in the area were compromised because I could have been rear-ended at high speed if I had instead hit my brakes and lost momentum in getting over to the shoulder. I was lucky to have reacted in the manner that I did in maintaining my remaining momentum. There was a very minimal shoulder available for me to pull off on. A Marin County sheriff was able to pull up behind me with flashing lights to warn drivers in the right lane to steer clear and avoid my vehicle. I had the vehicle towed to the nearest VW dealership. I had charged the car at an Electrify America station in San Francisco about an hour prior to the incident and the car had sufficient battery power (approx. 80%). I had no prior indicators that there was any problem with the vehicle. At the time of the incident, I had previously received all software updates and had the 30K maintenance service on [XXX], 2023 at Dirito Bros. Volkswagen in Walnut Creek, CA. FORWARD COLLISION AVOIDANCE

On 5/13/2023, I drove using adaptive cruise control (ACC). I traveled approximately 9 miles without issues and turned left into a parking lot. Upon turning, the break was engaged and which canceled the cruise control. I do not recall the exact speed it was set but it was over 50mph. I drive my VW ID.4 in B mode which is brake energy recuperation mode and brakes the car as soon as the foot is lifted from the accelerator. In the parking lot, I was driving at a slow rate of speed because it was a Saturday morning and there were lots of kids going to the Jui Jitsu gym and dance studio which was my destination. While navigating at this slow speed, I typical lift my foot off of the accelerator rather than tapping the brakes as it is an effective method in the B driving mode. I was turning right into a spot, my foot was on the accelerator because there is not coasting in this mode to turn into the spot. As I was turning the wheel hard to the right, my hand grazed the capacitive controls of the ACC and resumed the cruise control causing my car to suddenly accelerate at the previously set speed into the curbing. As mentioned, my foot was on the accelerator as I was pulling into the spot. When the car crashed into the sidewalk the impact caused my foot to depress further on the accelerator which caused a secondary impact into a support pillar further on the sidewalk. Fortunately, the pillar was there, otherwise, I would have crashed through the yoga studio directly behind the pillar which had a full class going. I also narrowly missed a father and son walking on the sidewalk to Jui Jitsu. I notifed VW and they analyzed the black box but claimed the accident to be my fault since the accelerator pedal was engaged, and I did not break until after impact. I do not dispute that.

Subject: Request for Investigation and Recall – Door Handle Failure on Volkswagen Vehicle Dear National Highway Traffic Safety Administration, I am writing to formally request an investigation and potential recall concerning a serious safety issue with my 2022 ID.4 AWD Pro S Volkswagen vehicle. The problem involves the door handles failing to open from either the inside or outside of the vehicle. I can provide video evidence showing that, while the vehicle is fully unlocked, with no child safety mechanisms engaged and my foot on the brake at a complete stop, the doors cannot be opened from either the front or rear seats. This issue occurs repeatedly—approximately four to five times per week—and has become more frequent since the “919” recall was performed on the vehicle. Despite this, the dealership has stated that the issue is a “non-issue,” claiming that other vehicles of the same model exhibit similar behavior. The inability to open a vehicle door in normal operating conditions is a serious safety concern that could endanger occupants in an emergency situation. Volkswagen’s response minimizes a clear risk to consumer safety. If the software controlling door operation cannot be relied upon to perform such a critical function, it should be removed or replaced with a mechanical mechanism that guarantees the ability to open the door at all times. I respectfully request that the NHTSA investigate this matter and determine whether a recall or corrective action is warranted. Thank you for your attention to this important safety concern. Please let me know if additional documentation or video evidence would be helpful to your investigation. Sincerely, [XXX] [XXX] [XXX] [XXX] 2022 Volkswagen ID.4 Pro S AWD [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

SW lockup, leading to malfunction of speedometer display (infrequent updates, roughly once per minute). Cleared by manual reset of central display system. Tuesday July 25. I-5 between Yreka CA to Ashland OR Starting about 3:50pm in Yreka, outside temp roughly 100 degrees Stopped to charge a Electrify America Station at Yreka Walmart, 30 minutes, starting at 16% change, ending at 68%. Initial rate of charge was 78KW. Came to a stop, Park button, stepped out of car. (AC shut down per standard as I stepped out of car. Attached HVDC charging, started to charge. Stepped back into car an attempted to turn on AC. Just started attempting to start car expecting that it would complain about charging in process. Dont recall the exact process, but did end up with AC active and Charging in process. Cancelled charging via button on charging station at 30 minutes (to avoid charging fees that started after 30 minutes) disconnected charging plug, got into car and activated high power drive system. center console showed showed main charging page. Dont recall seeing standard startup VW branding screen. Attempted to press ‘Climate’ button to turn AC back on, main screen, no response, screen stayed on ‘Charging’ screen. Press other buttons on center console, no response. Time on the display was stuck at 4:20pm. Put car into Reverse, drive (B) to get back onto freeway. Expected center console to deadman timeout, reset, it did not. did try about 5 second press and hold on the center console power button, guess I should have held longer. Drove for 15 miles to rest area along freeway. Center console stayed frozen, Steering wheel display did show speed and change of state of battery charge (charge/drain bars, battery current %, speedometer, etc) Center console stayed at 68% Speedometer locked up about 10 min later, cleared by manual reset 30 min later. into VW dealer/shop Sunnyvale CA 08-02-2023, NPF. Electrical system failure

POWER TRAIN,ELECTRICAL SYSTEM

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11762220

PRIMARY SAFETY DEFECT — SUDDEN STOP IN ACTIVE TRAFFIC: The vehicle suffers from severe, uncommanded electrical/drive-system shutdowns. While driving in live traffic, the ID.4 panics, flashes error alerts on the display, and abruptly stops within seconds. This sudden, violent stopping in active lanes creates an extreme safety hazard, leaving the EV dead in traffic with no warning and creating an immediate, severe risk of high-speed rear-end collisions, loss of control, and serious occupant injury. SECONDARY SAFETY DEFECT — ELECTRICAL & CHARGING SYSTEM FAILURE: Immediately upon picking up the EV post-warranty service on Aug 31, 2026, the dash displayed persistent error messages: "Your Charging Socket is not working." A VW Master Tech/Engineer at the dealership evaluated the vehicle, witnessed the failure, and explicitly warned that driving long distances poses a severe risk of complete vehicle disablement and becoming stranded on public roads due to total charging failure.REPAIR HISTORY & DAYS OUT OF SERVICE (OOS): The safety issues first occurred around Feb 2025 (towed to dealer). Since then, I returned the EV to the authorized dealer (Jim Ellis VW Kennesaw) at least 5 separate times for the same unresolved flaws. The vehicle has been OOS for warranty repairs for over 90 cumulative days, including 60 continuous days during the last visit. Dealer diagnostic logs confirm central control unit (CCU) replacements and extensive electrical repairs, yet the hazardous conditions remain uncorrected. MANUFACTURER RESPONSE: VW Group of America acknowledged this history under cases 07530936, 07526164, 07386099, 07334984, and 07709898. VW offered a 4.5k cash settlement, which I rejected as insufficient to address the extreme safety risks. VW has since refused a repurchase, ended negotiations, and closed the case without resolving the defect. This EV is dangerous and unroadworthy. My family and I have lost all confidence in this car.

I purchased this vehicle on [XXX] from Sunnyvale Volkswagen and have experienced ongoing issues with the electrical system, infotainment, safety features, and OTA software updates. In early 2023, all error lights came on while driving, followed by a complete electrical system failure. The car would not restart until the next day. In 2024 and again in 2025, the center screen went completely black, disabling the rearview camera, hands-free navigation, climate control, and car settings. Restart attempts failed. On Feb 26, 2025, I accepted an OTA software update, which failed. I tried again on Feb 27. When I returned to the car later that day, the update was still running. The next day, the car would not unlock with either key. I tried the manual key over an hour and hundred times, which eventually worked, but the keys still weren’t recognized. I replaced both key batteries with new ones from the store (one key was spare hence never used), but the issue persisted. After several days of the same issue and hours of failed attempts, I was finally able to start the car and drive it to Sunnyvale VW on March 3rd. They acknowledged the issue and replaced both key batteries again with OEM ones, which temporarily resolved the problem. On March 25, I was contacted by Stevens Creek Volkswagen to address two open recalls. I scheduled an appointment for March 31 at 9am and called on March 30 to confirm. Despite the confirmation and their having a note for a loaner from the manager, I was told upon arrival—after waiting 15 minutes—that parts were unavailable and they couldn’t perform the repairs. I was told I’d be contacted when parts arrive, the same excuse I’ve heard since the recall opened a year ago. My safety recall remains unresolved. This vehicle has proven unreliable and unsafe for my family. I am requesting a full replacement of this lemon with a brand-new vehicle as soon as possible. [XXX] [XXX] September 14, 2025 National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation 1200 New Jersey Avenue, SE Washington, DC 20590 Subject: Formal Complaint – Safety Defect in 2022 Volkswagen ID.4 (Power Steering and Brake Failure) Dear Sir or Madam, I am submitting this formal complaint regarding a serious safety defect in my 2022 Volkswagen ID.4, which resulted in a crash and a minor injury to my wife. This defect poses a substantial risk of death or serious injury to the driving public. On [XXX], my wife experienced sudden unintended acceleration, followed immediately by complete loss of braking and steering control. She was unable to avoid leaving the roadway and entered a ditch, sustaining a minor injury. The vehicle suffered fender damage. The vehicle was towed to our authorized Volkswagen dealership. The dealership’s technician documented that the power steering module failed. However, Volkswagen of America subsequently dispatched an engineer, who instructed the dealer not to communicate further with us. Volkswagen headquarters later claimed the power steering module failed only because the vehicle entered the ditch—an explanation directly at odds with the technician’s report and a witness account. Volkswagen has refused to accept responsibility, declined to cover medical or repair costs, and denied replacement of the failed power steering module. Our insurance provider also declined coverage, determining that the accident was caused by a mechanical failure. Legal and Regulatory References 1.49 U.S.C. § 30118 – Motor Vehicle Safety Act Requires manufacturers to notify NHTSA and vehicle owners when they determine a motor vehicle contains a defect related to motor vehicle safety. The failure of the power steering and braking systems clearly qualifies as a “safety-related defect” because it directly compromises steering and stopping ability. 2.49 C.F.R.
