Who was using the product(s)? The toilet was being used in a residential home by members of the household. 4. How were the product(s) being used? The product was installed in a standard home bathroom and used strictly for its intended purpose as a residential household toilet. The touchless wave-to-flush sensor was active and powered by standard batteries, operating automatically as designed by the manufacturer. 5. Were there any injuries that happened or may have happened while using the product? No physical injuries occurred. However, a potential injury or fire hazard existed due to the h
