SaferProducts.gov consumer incident report 20221215-54CA3-2147352018. Matched catalog product: Amazon Echo Dot with Clock (SKU: 810014301563). Source model text: Gen 1. Incident: Product was an Echo Dot Gen 1 that I had been using in my guest bedroom for quite some time. I just happened to be in an area I could see into the room, thank god as this room isn't used much, and I noticed the Echo power bank was smoking while plugged directly into a wall outlet. I quickly unplugged the Echo and found that not only did the Echo produce enough heat to actually burn my outlet and turn it black but the power bank itself began melting. This product, as noted by Amazon, is intended to stay plugged in and always on. It was not "in-use" at the time other than being plugged into the wall.

SaferProducts.gov consumer incident report 20200314-C62B4-2147374366. Severity: Incident, No Injury. Product: Amazon Echo Dot with Clock (SKU: 810014301563) (Amazon Echo Dot with Clock (SKU: 810014301563, S/N: G0914704007503JH) Smart Speaker with integrated digital clock and auxiliary audio output (1/8"). Power supply in question is marked "15W" and has the following identifying information on it: SCNA9CW1943G02S71, Manufacturer: Salcomp (Shenzhen) Co, Ltd., Model No: GP92NB SC P/N: AM0662 GG The power supply is marked with a UL Listed marking as well as a Double-Insulated marking among others.). Incident: I Purchase an Amazon Echo Dot with Clock (SKU: 810014301563, S/N: G0914704007503GH) recently. have had this item for a few weeks plugged into a stereo receiver to provide better audio via the 1/8" Audio jack. Today I touched the front panel of the receiver and it felt like I received an AC shock. I used a [REDACTED] multi-meter to measure the AC voltage between the receiver chassis and ground and found that I had 63 VAC at the receiver chassis. I then worked through all of my devices and eliminated them one by one until I finally unplugged the power supply for the Echo Dot with Clock. The hazardous voltage went away. I unplugged the power supply from the Echo Dot and measured from the outside of the coaxial power plug to safety ground with my meter. I measured 11 VAC in one direction and when I flipped the power supply around (It does not have a polarized plug) it measured 96 VAC. Either of these is enough to stop a human heart. The issue has been reported to Amazon but they did not seem to understand the gravity of the situation - may have been lost in translation. I work in the electronics industry so I am very aware of proper electrical safety and testing protocols. The power supply did not have any DC continuity between the prongs and the barrel of the coaxial jack so there must be DC blocking caps - but line voltage is definitely being passed from one of the
