Government / agencyIndexed Oct 22, 2026Source-linkedVerify source — SaferProducts.gov incident

SaferProducts.gov consumer incident report 20160714-C5162-2147416859. Severity: Incident, No Injury. Matched catalog product: Apple iPhone. Source model text: iPhone 5C. Incident: The Apple iPhone 5 series cell phones are using a lithium ion battery that are starting to fail. The batteries swell to sizes large enough to actually separate the glass cover from it's case. When I raised the concern to Apple they offered to replace the phone for the cost of the battery. This seem to bring my awareness level as they know there's an issue, but not willing to accept the responsibility for the design and parts used during fabrication. The Apple employee also stated that they would not just simply change the battery due to possible " thermal event". This even raised my concern level even higher. With all the problems with batteries and fires during recent months, I believe if they are willing to replace the cell phone for a third of the cost, either their profit margin is extremely high on their products or they are fully aware of this issue and are not willing to replace these batteries until something tragic happens.

SaferProducts.gov consumer incident report 20120717-B7AA5-2147465609. Severity: Incident, No Injury. Matched catalog product: Apple iPhone. Source model text: iPhone 3gS. Incident: I have an iPhone 3gS that I use for development and as a music player. When I came home today the case had come apart (screen was mostly popped out) due to the battery being extremely bloated. It looks like it will pop at any moment. There doesn't appear to be any damage to the motherboard but it is bowed a bit and the screws appear to have been popped from their holes. The device does not power on and this all happened in the course of a day. I was using the device yesterday and it was completely fine. It stays indoors (air conditioned) and it does not stay on a charger all the time. This is definitely a defective battery and if it explodes it could cause serious harm.

SaferProducts.gov consumer incident report 20121101-2A70C-2147462326. Severity: Incident, No Injury. Matched catalog product: Apple iPhone. Source model text: iPhone 3GS. Incident: We had upgraded our iPhones last year, and gave our old iPhone 3gs to our children. It has been working fine, until we found it this morning. We were charging it overnight, and when we went to get it, we noticed that the screen had popped out, and the case was broken. The battery had expanded considerably, and destroyed the case. I went online to check for other incidents, and I found many descriptions of this problem at apple.com forums and http://www.howardforums.com/showthread.php/1742082-iPhone-3GS-Battery-Expanded-and-Pushed-Out-Front-Glass among others. Our phone is out of warranty, so I doubt that Apple will do anything to help us, but there was no warning of this happening. It doesn't seem fair that a product would essentially self-destruct, even if it is to avoid the possibility of injury.

SaferProducts.gov consumer incident report 20130215-7246A-2147458779. Matched catalog product: Apple iPhone. Source model text: iPhone 3GS 16GB White for ATT, MC136LL/A. Incident: I had a Apple iPhone 3GS, out of warranty, where the battery was swelling. It caused the back of the phone to expand. I googled this and saw where other 3GS phone cases cracked, glass fronts came off, and some even exploded (battery exploded). I took it to an Apple store and they would only replace the battery for $150. An authorized Apple repair center told me to go to another Apple store ASAP because the expansion of the battery causing the hard plastic case to expand around the battery (very visible) indicated that the battery was seriously swelling and could explode. (the case seemed expand over a period of only a few days after noticing a dark spot on the screen, the first sign of the battery swelling) I took the phone to another Apple store and they acknowledged a battery problem and offered to sell me a replacement phone for $80.

SaferProducts.gov consumer incident report 20130420-48F53-2147456788. Severity: Incident, No Injury. Matched catalog product: Apple iPhone. Source model text: iPhone 3gs. Incident: iPhone 3gs was stored on a shelf with no liquid anywhere near it. When I looked at it today, it had basically expanded all by itself, so that the glass screen had separated from the body by about 2cm in some spots, though it was still adhered to it. I learned that there is a not-so-uncommon phenomenon of the batteries failing and essentially bursting. I consider this not only a product defect, but more imporantly, a serious safety hazard. I have no use for the iPhone at this point and it's not even salvagable by replacing the battery. I was informed by a friend who used to work for Apple that I should immediately place it in a [REDACTED] bag because the innards of the battery are TOXIC and that I should then take it to the Apple store ASAP as they actually have safes designated for this to store the items while awaiting disposal. They also stated that on at least one occasion in this person's store that they had to make a rush trip to the ER in response to just ONE of these devices behaving in this manner while in the store. THIS IS ABSOLUTELY UNACCEPTABLE from a company as reputable as Apple. Aside from their knowledge of this defect, this is just irresponsible and should result in some sort of repercussions for putting public health at risk. I have a small child and had she been using it at the time it decided to self destruct, I would be far more livid.

SaferProducts.gov consumer incident report 20151229-80F44-2147425345. Matched catalog product: Apple iPhone. Source model text: iPhone 5. Incident: Recently I have been having issues with my Apple iPhone 5, so i took it in to a nearby Apple store where a tech had diagnosed the issues stemming from a defect known as a swollen battery. She told me that the phone could not be repaired since they had been instructed not to take apart any phones with a swollen battery because of the risk of them exploding and causing harm to the technician, and that my only resolution was to replace the phone. I chose to do a little research into the issue myself before replacing the phone, which is when I discovered the issue has been pretty regular in the iPhone 5. In some cases individuals were left with burns and other injuries when the phones exploded in their pockets or leaked battery acid on them. I then took the phone to another Apple store where there, once again, the issue was confirmed to be a swollen battery, and she urged me to replace the phone because of the dangers it posed.

SaferProducts.gov consumer incident report 20170102-C1204-2147408599. Severity: Injury, No First Aid or Medical Attention Received. Matched catalog product: Apple iPhone. Incident: My Apple iPhone 6c was plunged into an Apple charger with an Apple charging cord which came with the original purchase of phone. I had noticed the phone would become extremely warm if it was in use while plugged in. This did not seem to be unusual. My phone was plugged in and lying on my bed. I got up to let my dog outside. After approximately 5 minutes I returned upstairs to my bedroom. I heard a hissing sound and thought it was the stool running. At the top of the stairs I could see an orange inferno covering my bed. I panicked and started to run and get the hose. Realizing it was unhooked and may have water in it that was frozen. I ran back upstairs for my phone to call the fire department an realized my phone would have been in the center of the inferno. Black smoke filled the room with the distinct smell of an electrical fire. To save my home from burning to the ground, I grabbed the side of my down comforter and flipped it over onto the flames. As I did that the fan of air to the flames whooshed out at me and caught the hair on the right side of my head on fire. I backed away to brush the fire in my hair off and noticed that covering the flames on the bed helped. I went into the hall for a breath of less smokey air and realized I must do this again if I were to save my home. This time I was more cautious as I flipped the other side of the comfort or over the flames. My beard melted to my face and all hair on my arms and chest was singed to the skin. Went for more air an
