Source model text: Swing By me. Incident: I have a Graco Swing By Me baby swing that was purchased back in 2012. We had it for my firstborn and then kept it for my second born who arrived in 2016. We have placed my second born in the swing many times and on a number of occasion,s one of the legs has unlocked on its own lowering itself on the front right leg. This has caused the swing to jostle and has made me nervous that it will collapse with my baby in the swing. I emailed the company who stated that because the product is no longer under warranty, they would not be able to fix the problem.

Matched catalog product: Graco DuetSoothe Swing and Rocker - Model 1893776. Source model text: Graco play yard 175646. Incident: We purchased a Graco play yard (model 1756464, manufactured 12/24/2009) a few months ago from Burlington Coat Factory. Once our baby was born on 4/7/11, we began using the changing table portion. There is a triangular cutout at each end of the plastic in the changing table. This area is dangerous - our son has caught and scraped hos toes in it. New designs in the store do not have this cutout. We have emailed Graco asking if this part been redesigned. While it appears that the worst that can happen is scraping of the toes/feet, it still shouldn't be designed so that a child can be injured.

High Chairs (1555)

Government / agencyIndexed Oct 28, 2026Source-linkedVerify source — SaferProducts.gov incident

SaferProducts.gov consumer incident report 20171124-4AD6F-2147395144. Severity: Incident, No Injury. Matched catalog product: Graco DuetSoothe Swing and Rocker - Model 1893776. Source model text: BLOSSOM /1812897 JJ. Incident: I am writing to inform you of an incident that occurred 11/23/2017 while my 18 month old daughter was sitting in a Graco Blossom High Chair (Model 1812897, Made 2013/03/09). She was sitting in the chair, and was strapped in and had the table in place. While I was feeding her, she leaned back and the entire top half of the high chair came off of the base, causing her to fall backwards on the tile floor. Being that this occurred on Thanksgiving, several other family members observed her falling. We checked her for injuries, and it appears that the back of the chair broke her fall, but only barely. We were very lucky that she was not seriously hurt or killed by this incident. It appears that what caused this incident is that the tabs on either side that hold the chair assembly to the base were aligned with the top holes on the stand and became loose. When I looked at the manual for the high chair, no where does it state how to secure the top part of the chair into the base, nor does it call out any warnings related to this item. I have also informed Graco of this incident.

SaferProducts.gov consumer incident report 20160701-65C0F-2147417333. Severity: Incident, No Injury. Matched catalog product: Graco DuetSoothe Swing and Rocker - Model 1893776. Source model text: Harbor Lights. Incident: I own the Graco Harbor Lights crib. On 6/30/16, the back of my 11 month old child's head became wedged in the corner of the crib, between the wooden slats (the side and back slats). I believe this is a serious design flaw with the crib that could result is serious injury. My child was positioned on her back and her head was pinned in a way that forced her chin to her chest, which I know is a dangerous position that can cause babies to suffocate. The pressure of being "stuck" also created large indentations on the sides of her head and took hours to normalize. Please call me anytime at [REDACTED] if you would like to discuss this incident.

Incident: On February 29, 2012, my wife was teaching a class at [REDACTED] and our one and two year old boys were in the Kids Club. My wife brought our 2010 Graco DuoGlider Double Stroller in for our one year old to take a nap in. Our two year old was trying to get something out of the undercarriage basket and proceeded to reach his head and arms into the basket. While in this position, he walked his body around to the side of the stroller to get a better reach on whatever he was trying to grab and his head got stuck between the frame underneath the back seat and the undercarriage basket. His head was stuck in an approx. 6-8 inch gap. The amazing kid's club staff realized what was happening immediately and jumped into action. It took 2 staff members and breaking the basket to remove him. During the minute and a half that he was stuck his airway was compromised and he stopped breathing. When the staff removed him he was blue and they were able to get him to come to within seconds and he began to cry. Their fast-acting, life saving efforts saved our son's life. After an ER visit and thorough check-up our son was fine with just some scraps and bruises.

Source model text: 9B08CAT. Incident: I have a Graco Playard model # 9B08CAT manufactured in 2/2010 and in use since 1/2011 (~16 months). When using the bassinet portion of the playard for my 4 month old son, he began crying like he was hurt. A different cry than usual. I went into the bedroom to check on him and pick him up and he was stuck. I couldn't physically pick him up. I then realized his head was stuck under the oval wall part of the bassinet and the 4 panel mattress has bunched up (the 4 panels overlapped each by about 1/2 inch - 1 inch) to create a space at the end of the mattress. He was stuck face down into the nylon bottom of the bassinet with the mattress pinning his head against the wall and the oval wall part above his head pinning it down into the nylon floor beside (not on top of) the mattress. I pulled the mattress further away from the wall and was able to maneuver his head out to free him. I am an emergency physician and saw that he was unhurt but also see the potential for this to fatally injure an infant. I wanted to report this so that if there other similar reports, a safety recall could be issued to prevent future injuries. I believe the problem is that the fabric of the mattress has stretched a bit over the 16 months of usage and now allows the 4 boards inside the mattress to overlap a bit which creates space at the edges of the bassinet. Because the bassinet has walls that shape the space into an oval, the oval wall can trap a baby's head. My mattress also with 16 months of use has bo

Incident: My 16 month old daughter got her upper arm badly stuck between a bar and a corner post in her Graco crib. That is the one slot between bars on the crib where the width of the gap varies from top to bottom. My wife had significant difficulty getting her arm out. My wife was very careful not to further injure my daughter's arm in the process. My wife finally had to push on the bar hard enough to break the crib to free my daughter. My wife called me at work and said that my daughter's upper arm was red and her lower arm looked misshapen compared to the other arm. She took her to the doctor to make sure she didn't have a broken arm. The provider that saw her concluded that she did not have a broken arm. But she said that if my daughter's bones didn't still have so much cartilage in them, then her bone would have likely snapped. No baby should suffer and come this close to a broken arm due to the proper use of a modern crib.

Unsafe for the baby and potential fire hazard. I contacted Graco to report the incident. They responded that the swing was past the one year warranty so would do nothing except offer a 20 dollar rebate on a costly new swing. Graco did not care that it was a potential fire hazard or that my grand baby or other children in my home could have been burned. Both my son and his wife were witness to the incident. Again, this swing is in like new condition and was only used four times.

Product: Graco DuetSoothe Swing and Rocker - Model 1893776 (Graco DuetSoothe Swing and Rocker, model number 1893776. This is a 2 - in - 1 infant swing with a removable rocker seat. The unit is a full-size baby swing with a metal frame and plastic housing, includes a padded fabric seat with a 5-point harness. The color is neutral gray/brown with a patterned fabric seat cover. the base and frame are primarily gray. The swing includes multiple motion speeds, vibrations, music and sounds, and can operate using either a wall power adapter or D-cell batteries located in the base. Approximate dimensions are 35" high, 33" wide, 28" deep when assembled. The rocker seat can be detached and used separately.). Incident: I am reporting a safety issue with a Graco DuetSoothe Swing and Rocker, model number 1893776. Prior to the incident, the swing began malfunctioning when powered by the wall adapter. The indicator light would turn on bright briefly, then dim and shut off repeatedly, and the unit would not start. It displayed behavior consistent with a "low battery" condition when plugged in. On the date of the incident, I installed new batteries in the unit while it was not connected to the wall outlet. Immediately after inserting the batteries and attempting to power it on, the swing began to emit smoke from the base area. The unit was turned off and all batteries were removed right away. The product has been used an intended and had not been modified. This incident created a potential fire hazard in a product intended for infant use. The unit is no longer being used due to safety concerns. No injuries occurred.
