Since new, TPMS fails to warn of a low tire unless the car is driven for 5 miles. The TPMS in this car is ABS-based, and each wheel does not have an individual TPMS sensor, rather the wheel speed is what determines if the car has a low tire or not. Brought to Honda several times, could not duplicate, says TPMS performs as it should. Recently had a flat, 3/4 tires were inflated to 37 PSI and the left rear had 4 PSI. Warning light did NOT come on. Drove on low tire for 2.5 miles before noticing I had a flat tire from a nail. AAA came, put the "donut" on the car, and within another 2.5 miles, the TPMS warning light came on, followed by "TPMS System Problem." Not the first time this happened, but I had to buy a new tire because I drove on it flat - which was ~ $267 in Costco for a Michelin Defender 2. Luckily, I did not drive too long of a distance that would've damaged the wheel, but this is problematic especially if you're on the highway. As you can tell, a 4-year-old car with 74K on it, I do a lot of driving. This needs to get rectified.

FORWARD COLLISION AVOIDANCE,LANE DEPARTURE

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11454419

The contact owns a 2021 Honda CR-V. The contact stated that while driving at various speeds, the vehicle would independently downshift without warning. The contact also stated that the failure would occur while the cruise control was activated. In addition, the contact also stated that the lane-keep assist would fail to differentiate between road salt and line marking on the roadway, resulting in inadvertent sudden downshifting. The contact's husband had taken the vehicle to a dealer where their concerns were ignored and no diagnostic test was performed. The manufacturer was not notified of the failure. The vehicle had yet to be repaired. The failure mileage was approximately 100. In addition to the above, the lane keep assist cannot distinguish between lines & cracks in the road, which is a big deal in Missouri, and an issue I fight with daily on roads I have no choice to travel on. And it's not downshifting that happens with the lane keep assist-it tries to correct, but the corrections if I don't fight them will either run me into the opposing lane or off the road.

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11682152

Sticky steering/stiffness in steering. Steering sticks in place and then when forced the car jerks quickly to either side. This has put my family and others at risk as it is hard to control the steering and causes sudden movements from side to side. No warning lamps or messages appear on the dash. The Honda dealer inspected the vehicle and determined the vehicle has an internal failure to steering rack and will require replacement. Replacement cost is $4,204.17. The vehicle is one year out of warranty. The large Honda steering recall is for model years 2022 and newer. Honda would not cover this repair under the current recall and will only do so if the recall is extended. I called American Honda to see if they would offer any goodwill and after their internal review they decided against any goodwill unless the recall is expanded as the vehicle is out of warranty. This is our first and last Honda. We will share our experience with our friends and neighbors - Honda seems to not understand what customer service means but they do know how to lose current and future customers.

With keyless transmitter in my possession…Intermittent Attempts to start the vehicle can be almost impossible due an extremely firm brake pedal with high resistance. First attempt.. extremely firm brake pedal with high resistance…pressing the start button… no results. Second attempt brake pedal will have a slight “give” .. pressing the start button only results in all dash warning/ inspection lights illuminating. Third attempt usually results in the vehicle starting normally. In an attempt to remedy the problem… I have replaced the start/stop button… problem still persists. Have read numerous articles and posts in forums and on the internet that other owners of various Honda models with the push button start switches experiencing the same issue. I have contacted my local Honda dealer… they state to date they have not had any customer reports of this problem … However… …. contacting an independent Honda repair shop reveals they in fact have had customers report this issue. When this happens… there is no warning… no illumination of DTC lamps on the dash… intermittent… but very concerning

LANE DEPARTURE,FORWARD COLLISION AVOIDANCE

10 months

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11459645

I was driving on a divided 4 lane road that offers a wide right side shoulder at around 43 MPH. I was traveling in the right lane. The lane markings are fresh and visible. It was late afternoon and sun was shining. The road surface was dry. There were no vehicles immediately in front of or along the side of mine. I had not activated the lane keeping assist or automatic cruise control features. Only the hazard mitigation/automatic braking and road departure mitigation features were operational. The car suddenly attempted to swerve left, away from the center of the lane being utilized. The automatic braking system also activated. I was able to simply "drive through" the malfunction without incident. This was the first instance I have noted of this kind. Due to the fact that it was late afternoon, there were shadows from overhead power line being cast onto the lane being driven. I have owned previous cars that sometimes attempted to track said shadow when the lane keeping assist is activated. That was not the scenario yesterday as LKAS was not selected. It was as though the sensors detected a pending pedestrian strike or other collision.

A car was way in front of me and had turned right into a driveway, before I was even a close and it stopped in the driveway entrance out of the road. I had my car turning away from the driveway knowing the car was moving slow in front of me. When I reached to pass this my ABS kicked on and brakes locked by itself and scared the pants off me!! My hands came off the steering wheel, my neck jerked forward and my glasses flew off my face at 50 MPH!!! I wasn’t even close to hitting this person. A few days later driving down the road a goose was on the same side right side on side of road as I’m rounding the bend my brakes lock up again!!! Again. The bird wasn’t even in the road!!!!! I had to pull over to read how to turn off this auto braking. I had to go to emergency room days later because my neck and shoulder pain was worsening. I went to ER last night. 3/23/23. Thank god just muscle damage. I was scared it was something worse! It’s just awful. Sometime you can see it say brake and nothing is in front of you. So I make sure I turn it off now. First incident was I think 3/12, when my car was doing 50 mph, second with the goose was 35 mph on 3/18.

The contact owns a 2021 Honda CR-V. The contact stated that while driving 60 MPH, the accelerator pedal was depressed; however, the vehicle failed to accelerate as intended. The check engine warning light was illuminated. The vehicle was driven to the shoulder of the road and turned off. The vehicle was restarted and driven to the residence. The vehicle was later taken to the dealer; however, the failure was not duplicated. The vehicle was taken to the dealer two times, but the failure was not duplicated. The failure reoccurred. In addition, several unknown warning lights were illuminated. The vehicle was taken back to the dealer where it was diagnosed with fuel injector and a misfire on cylinder number 1. The contact was informed that the fuel injector failure, and the contact was informed that the fuel injector needed to be replaced. The vehicle was repaired. The failure reoccurred three weeks later. The vehicle was taken back to the dealer where it was determined that a valve adjustment was needed in cylinder #2. The vehicle was being repaired. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 42,000.

It seems unreasonable that a model 4 years old and less than 63,000 miles will need $3,450 repair for a safety issue related to steering the vehicle. It occurs at all speeds. The steering problem appears to be very similar to the issue related to the 2023 models.

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11690793

I own a 2021 Honda CR-V with approximately 47,011 miles. I’ve noticed that when I hold the steering wheel perfectly straight, the vehicle pulls slightly to the right. This happens on flat, level roads and is most noticeable at speeds over 45 mph, especially on highways. At higher speeds, the drifting becomes more pronounced, and the car feels unstable. It seems like the vehicle is fighting against my steering input, which raises a serious safety concern, especially during long highway drives or in emergency situations. I took the vehicle to an authorized Honda dealership, and they diagnosed a failing steering rack, which they say needs to be replaced. The repair estimate is $4,478. This seems unreasonable for a vehicle with under 50,000 miles and no accident history or modifications. The steering rack is a critical safety component, and its failure at this mileage suggests a potential defect. I am filing this complaint to report the issue and urge NHTSA to investigate whether this is an isolated case or part of a wider problem. If others are experiencing the same failure, I believe a safety recall may be warranted. I also request that Honda be required to assist affected customers.

SERVICE BRAKES,FORWARD COLLISION AVOIDANCE

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11663387

My 2021 Honda CR-V has experienced incidents where it suddenly and unexpectedly applied the brakes while I was driving, despite: - No vehicle, pedestrian, or object is present in front of me - No driver input on the brake pedal - Not using cruise control at the time My car occasionally jerks or shows warning lights before stopping, sometimes without any warning. I suspect an issue with the Collision Mitigation Braking System (CMBS) or related driver-assistance components. These braking incidents occurred under normal driving conditions and pose a high risk of being rear-ended. My car's issues remain unresolved, and I feel unsafe while driving my vehicle. Two dealerships conducted four diagnostic tests (on 18, 19, 21, and 24 May 2025) to evaluate the Honda Collision Mitigation Braking System (CMBS) and the associated driver-assistance components. But no issues were identified during these evaluations. - 23 May 2025, I discussed my car’s issues with a Honda Case Manager (in California). He did not accept any responsibility for the reported Honda braking issues. - 27 May 2025, I filed a formal complaint with Honda Customer Service. Honda has not taken any action to address my car’s issues.

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11399794

The brand new CR-V had a brake system failure. The brakes were applied upon coming to a red light at an intersection. The vehicle slowed down. It did not come to a complete stop. I applied more pressure to the brakes and the vehicle continued to move forward. I had to put the vehicle in park in order to stop the forward movement. I then took the vehicle out of park and continued to apply the brakes. However, the brakes did not function properly and the vehicle continued to move forward. I put the vehicle in park again. During this time, several error messages popped up. When the light turned green, I put the car in drive and was able to pull the car to the side of the road. The vehicle still would not brake properly and continued to move forward after the brakes were applied. I put the car in park. I then recorded the error messages that popped up. Several error messages came up: 'brake system problem, tire pressure monitor problem, brake hold system problem, power steering system (eps) problem, vehicle stability assist (vsa) problem, hill start assist problem, adaptive cruise control problem, lane keeping assist problem, collision mitigation system problem, road departure mitigation system problem'.

FORWARD COLLISION AVOIDANCE

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11470565

While driving on a highway at approximately 60-65MPH, the vehicle in front of me began to brake. I was a safe distance from the vehicle in front of me. I also started to brake, but allowed the gap between us to close somewhat due to a vehicle following closely behind me. The Honda’s emergency braking system suddenly took over with MAXIMUM braking, causing items in my vehicle to fly around AND the vehicle behind me to nearly rear end me. I had NO control over the braking. My wife screamed at me, thinking that I had slammed my foot on the brakes, but it was beyond my control. I’ve asked my Honda dealer if the system can be adjusted but I was given no help. I understand the emergency braking system’s intended purpose, but the manner that this system has engaged (3 times) is unsafe. It’s not like I wasn’t paying attention, I was, and I was already braking. It’s actually scary the way it locked up. Yes, I know that the brakes didn’t actually lock up, they are antilock brakes, but it was like I was standing on the brake pedal. Have there been similar complaints about this? * The date below, 10/26/2021, is approximately the first time that this happened. It happened two additional times. It pretty scary when it happens.

ENGINE,FUEL/PROPULSION SYSTEM

53 months

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11701087

I am experiencing the problems that are described in investigation NHTSA CAMPAIGN NUMBER: PE25016. When driving on the highway, there have been multiple times (8-10) when the car stops accelerating and the check engine light starts flashing. I am not able to accelerate the car. If it turn the car off for 5-10 minutes, the check engine light stops flashing and I am able to drive the car again. It has always happened when I haven't driven the car for a while (either driving it for the first time for the day or after sitting idle at work for 8 hours), the problem happens within minutes of getting on the highway. I have taken the car to the dealership for service twice with this problem. The first time was in late August. They said there was no code with the check engine light and they could not replicate or find a problem. The most recent time was this past Friday (Nov. 21, 2025). I experienced the problem and immediately took it to the dealership. Because the check engine light was flashing, they were able to pull diagnostics and retrieve a code. They said a fuel check showed cloudy fuel and a spark plug was brown to copper color. They removed the old fuel and replaced all four spark plugs. This did not fix the problem, as I experienced it again this morning (Sunday, Nov. 23).

Dear National Recall Team, I am writing to report a recurring and potentially hazardous issue with my vehicle that has not yet been addressed through a recall. Since July 2024, I have experienced a problem with the steering wheel intermittently getting stuck, accompanied by a rattling noise from the left side of the vehicle. During a routine oil change, I asked the service technician to inspect the issue, but I was told everything appeared normal. Despite this, the problem has continued. I followed up with several phone calls but was consistently told that no issues could be found. I brought the vehicle back in again on Friday, where they replaced the brake fluid, yet once again reported no findings. Unfortunately, the steering issue and rattling noise persist, and I am increasingly concerned for my safety while operating the vehicle. When I contacted customer service, I was informed that only vehicles from model years 2022–2024 are currently covered under a recall for steering-related problems. As my vehicle does not fall within that range, I was told I do not qualify for assistance — despite experiencing similar symptoms. I respectfully request that this issue be investigated further, as it appears to pose a serious safety risk. I am happy to provide additional documentation or bring the vehicle in for inspection if needed. Thank you for your time and attention to this matter.

My cars automatic braking system engages when nothing is around. This has happened multiple times. One of these time I recall traveling with my child at a sped of 50-+mph. It was fearful for the both of us. This alone is dangerous, it could become deadly given different weather conditions causing loss of control of the car. I believe this issue will eventually cause someone injury and/or death. During this time my child was recovering from traumatic brain trauma. The dealership is waiting on a part the they feel should repair the issue. The emergency brake light flashed, beeped and slammed on breaks. I believe this is more than a sensor issue I believe it’s an electrical issue. The lights sometimes brighten instead of deeming in on coming traffic, there have been times that the usb port don’t work an then times that they do with the same charger, the windows an sunroof open on it’s on, the volume will max out an not turn down or off, the pixels on the screen become broken up(I have pictures) also when the car is idling the rear exhaust smalls like fumes from spray paint or something similar. I bought the car in May 2021 w/5miles - March 2023 w/about 7,500miles, I’ve had to call roadside assistance 3/4 times. I am a single mom with a young son who suffers from 4 rare diseases. I am unable to run out an get a new car. If these issues fix I’ll be on my own in the next 13 months with unnecessary expenses.

Total Repair Estimate Amount: $3,222.65 HPFP Assembly Replacement: $1,453.78 Fuel Pressure Rail Replacement: $1,193.80 Spark Plugs Replacement: $318.00 Engine Oil/Filter Change: $69.65 Safety Hazard Narrative: The internal mechanical failure of the high-pressure fuel pump causes a sudden loss of engine power and hesitation during vehicle acceleration, posing a severe traffic hazard and risk of rear-end collision when merging onto highways. Furthermore, internal HPFP failures on Honda 1.5L direct-injection engines can result in fuel dilution into the engine crankcase, risking major engine damage. Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11678168

The power steering rack failed making it difficult to steer the vehicle. The problem steadily grew worse over several months and was verified by a dealer service center. Clearly, faulty steering is a safety risk and one that Honda is aware of, having previously recalled 1.7 million vehicles similar to mine. The dealer merely changed out the power steering rack rather than actually investigating to determine the specific problem (i.e. opening the power steering gear box). The following is a synopsis of my complaint: 1) Honda produced a defective part which clearly impacted the safety and performance of my vehicle after only 53,000 miles. 2) Honda refused to acknowledge the defect in this particular vehicle despite recalling 1.7 million others. 3) Honda advised that if the defect is acknowledged in the future, only vehicles repaired by a Honda dealership may be eligible for compensation. 4) Honda technicians, upon performing the necessary repair did NOT investigate whether the problem was caused specifically by the same gearbox issue that warranted recalling the other vehicles. The faulty power steering rack was not inspected by anyone. It is currently in my possession. There were no warning lamps or messages. There was difficulty steering the vehicle. There was a "sticking" sensation in the steering wheel which grew worse and eventually kept the vehicle from being able to be steered smoothly at speed.

While Driving on freeway, engine suddenly stops. This has happened four times now in the last two years one time was almost deadly as we were surrounded by semi’s and lost power completely except for the small amount of hybrid power. I had to quickly get to the side, barely missing a semi truck. 2021 Honda CRV hybrid. I have taken it into Bountiful Honda in Utah after each episode, the first time they did some adjustments it was about $600. The second time they just kept it for six weeks and couldn’t find anything wrong. Third time was the same and this last time they changed the fuel injectors. But we’re scared to death to drive our car. We’ve been renting cars every time we have to go more than an hour away from our home or get on the freeway for very long. We are hesitant to trade it in because we don’t want them to turn around and sell it to someone else that would not be aware of the problem. But we were able on the last episode to retrieve a code with a little machine we purchased. But that code just meant severe engine failure. The dealership kept my car for almost 2 months on the third episode and drove it over 2500 miles and were unable to get it to replicate. It seems to happen every 3 to 4 months with normal driving. I have records for everything. This car is a hazard to gave on the road. I just can’t afford to just buy a new one without trading this in and I just can’t do that with good conscience.

ELECTRICAL SYSTEM

53 months

Government / agencyIndexed Oct 9, 2026Source-linkedVerify source — NHTSA complaint #11701089

Component or system involved: Electrical system / infotainment unit / audio system / driver-assist camera systems. Yes, the affected components are available for inspection upon request. Safety risk: The incident occurred at highway speeds, which created a potential safety hazard because the infotainment system suddenly shut down, the audio system emitted a loud “pop” sound, and driver-assist features became unavailable. The distraction and temporary loss of information could increase crash risk. Reproduction: The issue has not been reproduced yet and has not been confirmed by a dealer or service center at this time. Inspection: The vehicle has not yet been inspected by the manufacturer, dealer, police, insurance, or any other agency. Warning signs: No warning lamps or messages appeared prior to the failure. The issue started suddenly while driving. What happened: While driving at approximately 65 mph on a major highway with cruise control active, the infotainment system suddenly shut off for about five seconds. At the same moment, the audio system emitted a loud “pop” noise similar to a small explosion. After that, the screen restarted and remained stuck on the Honda logo for around two minutes. When it finally rebooted, the clock reset itself to UTC instead of the correct local time. All of this occurred while the vehicle was in motion, which created a distraction and potential safety concern. No previous warnings or symptoms were present before the event.

What component or system failed? Is it available for inspection? The component is the Steering Gearbox Assembly / Rack and Pinion. The vehicle is currently in my possession and available for inspection upon request. How was your safety or the safety of others put at risk? While driving at highway speeds (40–70 mph), the steering wheel "sticks" or "catches" in the center position. To change lanes or follow a curve, I must apply sudden, extra force to "break it loose," which results in a jerking motion and significant overcorrection. This creates a high risk of losing control of the vehicle or unintentionally departing the lane into other traffic. Has the problem been reproduced or confirmed by a dealer? I have presented the vehicle to an authorized Honda dealership on two separate occasions for this specific issue. Both times, the dealership stated they were "unable to duplicate" the problem and claimed the steering was "normal," despite the symptoms matching a known national defect. Has the vehicle or component been inspected by the manufacturer, police, or others? No, it has only been inspected by the authorized Honda dealer technicians. No police or insurance reports have been filed as no crash has occurred yet. Were there any warning lamps or messages? When did they first appear? There are no warning lamps or dashboard messages. This is a mechanical friction failure within the gearbox, not an electronic one. The symptoms first appeared roughly 2023 and have progressively worsened with time and heat.

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11755121

Description of Incident: Immediately after purchasing this 2021 Honda CR-V Hybrid brand-new at an authorized Honda dealership in Maryland, the driver’s seat frame exhibited abnormal mechanical play. The seat shifts and rocks forward and backward during standard acceleration, braking, and turning maneuvers. The issue was brought to the selling/servicing Honda dealership in Maryland shortly after delivery while under factory warranty. However, dealership service staff refused to repair or replace the frame assembly, claiming the play was within normal specifications. The defect has persisted throughout the vehicle's operation. Safety Risk: The failure of the seat structure to remain rigidly locked in place compromises the integrity of the vehicle’s primary driver restraint system. Unintended seat shifting during sudden stops, sharp turns, or a collision alters driver posture, increases driver distraction, and potentially impacts the proper alignment of the seatbelt and airbag deployment zones. Confirmation & Inspection History: Reproduced/Confirmed: Yes, the physical shifting occurs continuously during operation and was directly presented to the servicing Honda dealership. Inspected By: Authorized Honda dealership service personnel in [XXX]. Symptoms & First Appearance: Mechanical rocking and play present from the day of purchase at 0 miles. SERVICE BRAKES,FORWARD COLLISION AVOIDANCE

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11670462

I am reporting a serious safety issue with my 2021 Honda CR-V involving the Collision Mitigation Braking System (CMBS). On multiple occasions, the vehicle has applied the brakes suddenly and forcefully without any obstacle or vehicle in front of me. THIS HAD HAPPENED OVER 20 times since i bought the CAR on March 10 2024. The most recent incident occurred while I was driving at highway speed on a clear, dry day. The CMBS system engaged abruptly, causing the car to decelerate rapidly and SLAMED BREAKS automatically. There were no vehicles, pedestrians, or obstructions ahead. This unexpected braking could have easily resulted in a rear-end collision, endangering me and other drivers. I have kids in the back sit and the car behind us almost hit my car. I am afraid the result might have been much worse if there was TRUCK behind my car. I took the vehicle to the Honda dealership, but they were unable to replicate the issue and did not offer a fix. They also mentioned that unless the problem could be duplicated, any diagnostic or repair fees would not be covered, despite this being a known issue under investigation by the NHTSA. I am aware that the NHTSA has an open Engineering Analysis (EA24-002) into this very issue affecting millions of Honda vehicles. I’m deeply concerned that this problem continues to put drivers and passengers at risk and believe a formal recall is necessary. I request that this issue be escalated and that Honda be held accountable for a fix before someone gets seriously injured or killed. Please contact me if additional details or documentation are needed.

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11734701

While driving my 2021 Honda CR-V at highway speeds, the steering intermittently binds and becomes difficult to move, creating a dangerous “sticky steering” condition. The steering will momentarily resist normal input, and when I apply additional force to correct it, the wheel suddenly releases, causing the vehicle to jerk or swerve within the lane. This makes it difficult to maintain smooth, controlled steering and creates a serious safety concern, especially at higher speeds where precise steering is critical. The condition is unpredictable, unnerving, and significantly affects my confidence in safely operating the vehicle. This defect creates an especially concerning hazard because I frequently transport my two young grandchildren in this vehicle. When the steering binds and then abruptly releases, it compromises my ability to maintain stable lane control and increases the risk of losing control or being involved in a collision. This is not merely an inconvenience—it is a clear safety issue that makes the vehicle feel unsafe to drive. The symptoms my vehicle is experiencing are identical to those described in Recall 24V-744 involving sticky steering in Honda vehicles. Although my VIN is not currently included in that recall, this same defect has been widely reported by other 2021 Honda CR-V owners. I understand the defective steering gearboxes were identified in vehicles produced beginning in February 2021, and my vehicle was manufactured in August 2021, which falls within that timeframe. Based on the identical symptoms and known defect history, I believe my vehicle is affected by the same steering gearbox issue and should be included in the investigation and recall population.

ELECTRICAL SYSTEM,ENGINE,FUEL/PROPULSION SYSTEM

52 months

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11726658

2021 Hybrid CR-V - within the last 10 months the vehicle has twice lost engine power while driving on expressways with just enough time to pull off the road. In the same period of time, at least 6 times, when initially starting the engine, the engine runs roughly and will not accellerate. So far, this has happened in cold or cool weather but not extremely cold weather. In an effort to restart the system, we then shut the system off, wait a few minutes and try restarting. Occaisionally, the engine will start normally after this "reset". More often, if the "reset" doesn't work, we shut off the heat/cooling system (to keep the system from calling for the engine power) and restart the system in EV Mode. The car will begin moving in EV Mode. The engine then starts and runs normally when accellerating beyond the EV system's capacity to accelerate. We understand this problem does not register in the car's computer as a "fault" once the car's engine operation returns to normal. Therefore, the dealers are unable to diagnose the problem. We have read of many other people having the same experience without any satisfactory correction from the dealerships or mechanics. Because this is an intermitant and unpredictable failure, and because the engine and system operates normally for long periods between incidents, the error must be computer/electrical and not related to the fuel pump. We have also replaced the aged engine battery and cleaned the cables. This did not correct the problem. Any assitance in resolving this issue is appreciated. We are concerned that the system will fail completely at the most inopportune time, leaving someone stranded on the highway or being a road hazard on a highway.

Powertrain / transmission failure

POWER TRAIN

56 months

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11721063

While ascending a steep hill in San Francisco, the engine began operating roughly and the check engine light came on and began flashing. Pulled over as soon as possible, let the engine cool, and confirmed in the operator manual that the flashing light indicated severe misfire and possible loss of engine power. After cooloff and restart, the flashing check engine light went off, but a scanner revealed a pending P0302 error code, indicating a misfire in cylinder 2. On the first restart of the engine, it started roughly before settling down, and I could hear loud water sloshing. On the short drive home, I could continue to feel the engine running roughly on each startup cycle (periodically on the drive since this is a hybrid), indicating to me a blown head gasket. In my research, this seems to be a common issue with this engine or model year (see NHTSA Preliminary Evaluation PE25016 or Bissell v. American Honda Motor Co., Inc. et al). I'm confident that if I continue to drive it, another misfire like this on a hill in San Francisco could easily cause a loss of power, steering, or braking action. I will be taking it in for inspection at a Honda service center. The service representative at my local service center (Berkeley CA) indicated this is a common issue on this car that they see regularly. No prior mechanical issues or engine roughness with the vehicle, or anything that would've indicated an issue prior to this misfire and leak. Regularly serviced as a leased vehicle from Honda dealership until it was purchased 2 years ago from the dealership with 70,000 miles. No major maintenance since purchase besides oil changes, rear differential fluid changes, air filters, and tire replacement.

What component or system failed or malfunctioned, and is it available for inspection upon request? The A/C compressor seal may be leaking under certain conditions. The component is available for inspection by an authorized Honda dealer or service center. How was your safety or the safety of others put at risk? If the compressor fails completely while driving in hot weather, the lack of air conditioning can cause discomfort and distraction to the driver, potentially increasing risk of unsafe driving conditions. Has the problem been reproduced or confirmed by a dealer or independent service center? The issue has not been reproduced, but Honda has issued an extended warranty for this known problem. A dealer in Puerto Rico stated that action is only needed if the system fails completely. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The component has not been formally inspected yet; the dealer only provided a verbal assessment. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warning lamps or messages appeared. The system is still operational but may exhibit leakage over time. What happened? In your own words, tell us what happened. The vehicle is a Honda CR-V Touring 2021. I received a notice regarding an extended warranty for the A/C compressor seal. When visiting a dealer in Puerto Rico, they stated that the issue only requires action if the A/C fails completely. I am reporting this because the extended warranty indicates that any potential leakage should be inspected and addressed, and I want the vehicle properly checked to ensure the compressor seal is not failing.

License Plate: [XXX] (CA) Vehicle: 2021 Honda CR-V Hybrid Mileage: 102,700 Description of Incident: While operating the vehicle at highway speeds, a "Engine Temperature Near High" warning suddenly appeared on the dashboard. I immediately heard a loud "gurgling" or "water pouring" sound from the dashboard area. I was forced to navigate through traffic to the shoulder to prevent a total engine seizure and avoid a high-speed collision. I was able to restart and limp the vehicle to Tracy Honda (Dealer), where a certified technician performed a diagnostic (Repair Order #XXX) and confirmed a blown head gasket with coolant leaking into the combustion chambers. Detailed Defect Information: The vehicle is experiencing a premature failure of the head gasket, a component designed for the life of the engine. This failure allows coolant to breach the internal engine seals and enter the cylinders. This specific issue is currently being litigated in the consolidated class-action lawsuit Bissell v. American Honda Motor Co., Inc. (Case No.XXX in the Southern District of California. Despite following maintenance schedules, the engine hardware failed, creating a sudden loss of thermal management. Safety Risk: The sudden onset of overheating while in high-speed traffic creates a significant safety risk. The "Engine Temperature Near High" warning provides insufficient time for a driver to safely exit a highway before the risk of catastrophic engine failure or fire becomes imminent. This is a documented manufacturing defect in 2017–2022 CR-V models that poses an unreasonable risk of accidents or being stranded in dangerous traffic conditions. Despite proper maintenance at a Honda dealership, I was informed of internal engine sludge. The issue appears linked to the engine not reaching optimal operating temperatures—leading to oil degradation and sludge formation over time. This matches problems detailed in official Honda Technical Service Bulletins (TSB 21-013 and TSB 22-087), yet I was denied warranty support, even under a Goodwill request. Honda regional representatives and mechanics provided no meaningful analysis or assistance and defaulted to denying support without adequately addressing the root cause. This is deeply concerning, particularly since multiple owners nationwide have reported similar problems. STEERING

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11756968

I love my Honda CRV however, I recently have experienced what I call "Sticky Steering". I purchased this vehicle brand new from Jones Honda in Lancaster and have followed all the service suggestions for the most part to the "T". I used it for my Real Estate job along with filling in the gaps with driving for lyft. About 4 months ago I noticed the steering sorta wanted to stay in the position it was in when I wanted to slightly adjust the steering it was "Stuck" and I had to extend more pressure than normal to get it to move. Than all of the sudden it moved to another spot but got stuck in that spot. Needless to say this really concerned me. I than searched online to see if others are having the same problem to find out they are! When I checked the recall on my vehicle I found out that 2022-2024's were recalled but my 2021 CRV was not. This is definitely a safety issue because if a person would "Over steer" to get it unstuck the car could jolt into another lane of traffic or even go off the road and have a crash. I found out that there are many people having and complaining about this same issue online. I do not have the money to make the repair myself so that is why I am reaching out. Maybe my vehicle along with other 2021 models can be repaired and hopefully save someone from being killed. It's very unnerving when it occurs. The problem is I never know when it's going to get stuck. I was hoping to drive this CRV till the wheels fell off but now I am forced to drive with extreme caution every time I get behind the wheel for fear of not knowing when the steering will "Stick". Please help me get this repaired. From my understanding there is a faulty electric motor that controls the steering that for some reason does not operate properly.

FORWARD COLLISION AVOIDANCE

50 months

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11684543

About a year ago, I was driving at speeds of approximately 10-15 mph when my car braked and immediately brought me to a dead stop despite no obstacles being present in the roadway. It appears the automatic emergency braking system malfunctioned. Had there been a car behind me, it likely would have caused an accident. On August 15, 2025, I was driving on the freeway using the adaptive cruise control when traffic in front of me slowed, engaging the automatic emergency braking. Traffic in front of me cleared almost immediately but my car was still very rapidly decelerating. I pressed the gas pedal, but the vehicle continued to bring me to almost a complete, immediate stop on the freeway (I had been traveling at 70-80 mph prior to the stop). I feel incredibly fortunate that I did not end up in an accident due to the traffic behind me and that no one was injured or killed. I pulled off onto the shoulder when it became clear that I was not going to be able to quickly regain control of my vehicle. Driving a vehicle without being able to properly control it was an absolutely terrifying experience. I was bracing myself for impact from behind as I desperately tried to regain control and accelerate back up to the flow of traffic. I am certain that this malfunction will result in deaths if not addressed. On August 23rd, 2025, I was driving on a highway at approximately 60 mph when the car in front of me slowed in preparation for a right turn off the road. I had disabled the CMBS feature at the beginning of my drive, yet the car still braked without me hitting the brake pedal. It was not an immediate, abrupt brake as I had experienced in the other two previously described incidents, but it was still highly unnerving to feel another loss of control of the vehicle, especially since the system was supposedly disabled.

ENGINE,LANE DEPARTURE,FORWARD COLLISION AVOIDANCE

Government / agencyIndexed Oct 8, 2026Source-linkedVerify source — NHTSA complaint #11706706

Dear NHTSA, There needs to be an immediate recall on this (VIN#: [XXX] ) 2021 Honda CR-V Hybrid Motor Nightmare. Despite proper maintenance and care, the vehicle has experienced persistent problems that stem from known defects. The engine has a fatal flaw with the head gasket. The high compression turbo engine stretches the head bolts, lifts the cylinder head just enough to blow the gasket causing coolant to leak into the cylinders and mix with the oil and destroys the bearings. Along with oil dilution where gas washes pass the piston rings. This is a widespread problem affecting this model as indicated by the numerous technical service bulletins. The issue appears to occur during the 20,000 to 30,000 mile mark where it begins to experienced recurring engine performance issues, misfires, loss of power, and excessive radiator fluid consumption resulting in premature engine wear and sludge buildup. Honda sites have pointed out: "This can happen when the car is frequently idled or driven for short commutes, which can prevent the oil from flowing properly" and my commute is a 3 minute drive and 5 minute to the market. [XXX] "it’s a well documented issue and Honda America is aware of it that they’ve released an article about it for mechanics" One customer wrote "I have a 2021 Honda CRV and the engine had a terrible knock" [XXX] Please do something immediately! Several Mechanics acknowledge these widespread issues, the Lane Manager that advised me to reach out to the manufacturer pointed out that this is a “common issue”, and Your Website highlights that you all have already received similar recall campaigns for the engine oil and gasoline dilution issue that occurs approximate at the 22,500 mileage. While driving a 2021 Honda CR-V Hybrid, the vehicle has intermittently and without warning lost the ability to accelerate. When this occurs, depressing the accelerator pedal — even fully to the floor — produces no response, and the vehicle simply coasts and loses speed. This has happened on three occasions — January 13, 2025; December 16, 2025; and June 10, 2026 — including at highway speed. Each occurrence is documented by a Honda dealer service invoice. In each instance, the check engine light began flashing immediately before the loss of power. The only way to restore normal operation was to bring the vehicle to a stop, turn the ignition off, wait, and restart. Restarting restores power but also clears the check engine light and any stored diagnostic trouble codes. Safety risk: Losing motive power without warning, particularly at highway speed or while merging in traffic, creates a serious risk of a rear-end or loss-of-control collision, because the vehicle cannot accelerate out of danger. The affected systems appear to be the power train and hybrid propulsion system, possibly originating from the engine given the flashing check engine light, which typically indicates an active engine misfire. The exact failed component has not been confirmed (UNKNOWN). The problem has NOT been reproduced or confirmed by the dealer during inspection. The vehicle was taken to an authorized Honda dealer for each occurrence, but because restarting the vehicle clears the fault codes, the dealer found no stored codes and could not duplicate the failure during a test drive. The vehicle has not been inspected by the manufacturer, police, or insurance. The vehicle is available for inspection upon request. This matches NHTSA Preliminary Evaluation PE25016 regarding intermittent loss of motive power in MY 2020–2022 Honda CR-V Hybrid vehicles.

On [XXX] at approximately 7am, I was driving eastbound on [XXX] in Missouri, just west of Waynesville, MO. Weather was clear, not a lot of traffic. My sunroof glass exploded. There were no cars around me, I had not seen anything fly out from anywhere and hit the sunroof and I don't recall hitting any type of large bump or pothole prior to the glass shattering. The noise it made was very loud; like an explosion. The sunshade was open at the time and the tempered glass shattered all over the inside of my car. Despite shattered glass everywhere, I was not injured. There were no passengers in the car at the time. Since most of the glass fell inside the cabin, I closed the sunshade so no more would fall on me and pulled over in Waynesville. I was then able to drive on to my destination in Florissant, MO with the sunshade closed. Once there, a mechanic broke out the rest of the glass for me and installed crash wrap until I could get the window fixed. He also vacuumed out as much glass from the cabin as he could. I contacted my insurance company, sent photos and a claim was started. In the end, my insurance company took care of the claim, minus deductible. There were no warning lamps, messages or other symptoms of the problem prior to the glass exploding. I had also just had a multi point inspection with my tire rotation and oil change just 3 days prior at Pinegar Honda in Springfield, MO and there was no notation that the glass was cracked or otherwise compromised in any way. I don't know if this is a problem occurring with Honda CR V's or if it's just a fluke occurrence with my particular vehicle. The mechanic was unable to determine what caused the glass to shatter. I am reporting this just in case this is happening with other vehicles and the problem is more pervasive. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

FORWARD COLLISION AVOIDANCE

I was driving on a clear mid-day with very little traffic. There were no cars in front of or beside me. There were no people or animals anywhere in sight. I was traveling west at 45MPR when suddenly the car slammed on the brakes. The warning light and beeping came on. I was panicked thinking I hit something but there was nothing/no one in sight. This happened in August 2026. I am not sure of the exact date. I just thought it must have been a fluke (bird flew by or some other random event) About a week-2 later on August 30th, I was traveling with a friend on the highway going 70MPH at the time when once again the brakes slammed on. It was so sudden and without warning that we both screamed wondering what had happened. Again - no cars, people, animals in sight. Fortunately both times, no one was behind me or I likely would have been hit. The automatic braking mitigation system is malfunctioning and is a very serious safety concern. On September 2, I took my car to Roush Honda to have it checked out. After keeping my car for 5 hours, running it through the computer evaluation and taking it for a drive at different speeds, they could not duplicate the problem and told me there was nothing they could do. I informed them I wanted the system turned off as it is unsafe to drive my car as is. They told me I could turn it off each and every time I started up my car. I do not feel that I should have to to this. I paid for a safety feature that is clearly malfunctioning but having it on is like playing Russian Roulette with my safety and the safety of my passengers. No one other than the dealership has inspected the vehicle. The brakes slammed on hard at the same time that the warning sound and orange warning came on. It is truly terrifying to drive the car with this problem so I have forfeited this safety feature by turning it off each time I start up my car afraid that it could malfunction again causing potential injury.

1) On 3 occasions the Automatic Emergency Braking (AEB) has self activated and the car attempted to come to a complete stop. The 1st incident, 10/19/24, happened while driving 35-40 mph while changing lanes, a red BRAKE light and alarms sounded bringing the car to a complete stop. While there was traffic around me, no one was directly in front of me or close enough to have required this to activate. A vehicle traveling behind came inches from rear-ending me. The 2nd incident, 11/9/24 concerns me the most, happening 3 weeks after the first incident while on the highway traveling 65 mph. I was in the left lane of multi-lane traffic. While there were vehicles two lanes to my right, there was no one directly in front of me by 100 yards. Again, alarms, red BRAKE light, and the car proceeded to come to a complete stop. The car felt like it was floating and for a moment, I felt like my life was over as the car was out of control. The 3rd incident, 12/9/24 happened 9 days after it was in for service, while traveling 30 mph in town. 2) The 1st incident avoided an accident because of the alertness of the driver behind me. The second incident, at 65 mph, is of the most concern, as for a few moments, I felt like my life was over. This incident is also causing me to have PTSD symptoms while continuing to drive, it has required me to see an MD. 3) An appointment was available about 3 weeks after the second incident. They were not able to replicate it or find anything wrong with the vehicle. After speaking about my concerns after the 3rd incident to the dealership, they have recommended that I turn off the safety equipment. While I have spoken with Honda America, they do not want to get involved unless there is a recall by you, the NHTSA. Nothing has been reported to the police but I have spoken to my insurance agent regarding liability if I turn the safety equipment off. 4) BRAKE light and alarms occurred at the same time as the braking.

BACK OVER PREVENTION

Safety defect in my 2021 Honda CRV. Until 11/23/2025 the rearview camera had perfect view. 11/24/2025, camera view became obstructed by the camera Lense black rim in the permanent housing. On 11/28/2025 saw my local mechanic. Told after visual inspection that the lense had slipped down in the housing. This would have to be repaired by Honda. Appt.11 AM on 12/18/2025 at Crown Honda Pinellas Park Fl. I viewed Honda manual for 2021 Honda CRV camera problems. Had to be taken to a Honda dealership for repair. Honda forums, and Honda repair technician blogs stated backup camera lense is a defect, has to be repaired by Honda. At Crown Honda on 12/18/2025 at 10:45 AM, I was checked in by a manager. This guy ran the outside car take in. He looked at the problem in driveway. After 15 min, I was told its “Normal” he said “I looked at the same year in lot and it looks the same.” I told him the camera lense housing is loose and needs repair,“ no its not” he said. I explained the backup camera lense dropped further since 11/24/2025 and almost resulted in a pedestrian injury twice. Backing out of my drive way and in a shopping center parking lot. Both times from the rear right side of the car, a known blind spot in Honda CRVs. Both times the camera did not see the cross-traffic movement. I insisted a mechanic look at the car. Another shop manager comes and says its normal. I showed both of them the proof it is not normal, my obstructed view is a safety concern. At 12:07 they finally agreed to have mechanic # 971716A look at it. The second manager says “nothing is wrong.” I said, “if an injury occurs which is a result of this obstructed view, I will hold you/him accountable.” I said” check the housing holding the camera” he said “No, and the truth is; not enough people are complaining for Honda to pay for this repair”. I am truly concerned this is a serious safety hazard that can result in injury or death due to an obstructed view of rear camera.

ELECTRICAL SYSTEM,ENGINE,FUEL/PROPULSION SYSTEM

Hello NHSTA -- My Honda is a 2021 CR-V AWD Hybrid. Two very unsafe events to report which occurred within a few weeks of each other, apparently from the same cause. First event, on 27 Feb 2026, I was driving on a two lane county road, traveling about 40 mph, I was in the lead with 2 or 3 cars behind me. Suddenly, on a fairly level grade, I noticed that the car was gradually slowing down; I was not touching the brakes, my foot was on the accelerator. When I pushed it to speed up, there was no effect, no change in the engine sound. I put on my flashers, and scanned for a place to pull-over, but there was no shoulder. My speed had reduced to around 5 mph. The check engine light flashed I believe, but it did not stay on. In about a hundred yards I was able to locate a broad drive-way entrance to pull in to and I was able to let the cars behind go by. Turned the engine off 2 or 3 times. On the 3rd time, the car moved a little bit. When I put it in reverse, it moved back a little bit. So, when the road was clear I put it in drive again and it took off like normal. I drove directly to my local Honda shop. The kept it for a couple of days. They couldn't find anything, and said that no service codes had been recorded in the system. Given the age of the battery, we put in a new one. And the car ran fine from that point. Then, second event, a few weeks later on 30 March 2026, I was driving on an Interstate (3 lanes each way), traveling around 55 mph, flat terrain. Suddenly, the car began to lose power and gradually slow down, I had no acceleration or control over it. I was in the middle lane, Put on the flashers and drifted into the right lane, the other cars slowed for me. Got over to the shoulder. Check engine light was on steady. Called Honda, per their instructions from the first event. They told me to have it towed. Bottom line, Honda said that the intake valves were too tight. See attached worksheets. Clearly a factory build defect. Valves are not checked until 90K.

Head gasket failure. 79,600 miles. Car consumes coolant. Coolant is entering the firing ring when car sits after being driven a distance of 5 miles or more and sits for about 8 hours, vehicle starts with a misfire and makes a small amount of white sweet smoke out of the exhaust pipes even in 85 degree weather. Car smooths itself out after 10-30 seconds. No OBDII or Honda specific codes. Spark plugs, ignition coils, and spark plugs again since 75K. Over $2,000 into this car in repairs. Refilled coolant reservoir 3 times since April 8TH (3 full jugs of Peak Asian blue from Advanced Auto Parts). I want justice for this car and other Honda 1.5T vehicle owners as many of them have this happen at the 80K mark and they’re being quoted $10K for an engine and $5K for a head gasket. There are several people with Accords, Civics, CR-Vs equipped with the 1.5T engine that have coolant leaking into one cylinder. Oil looks fine and not milky. And besides the coolant being low, it looks fine (blue/aqua). Only when the car sits. Car was brought to Rivera Honda in Port Jeff and they couldn’t find any issues. Car is consuming coolant. No leaks. It’s being burned when you start the car after it sits. Gushing liquid noise when car starts or with idle engine stop activated and car restarts.Design flaws on cylinder head has grooves that allow coolant to flow on top of the head to cool it. Failure point according to a Honda mechanic on YouTube. Second flaw is poorly designed head bolts that are too thin and are brittle, not making a good seal as the vehicle ages. Parts are not expensive at all, but the man hours needed to replace this are the killer. 4.5 years old with 80K, it’s Honda. It’s supposed to run forever. Not die under 100K. I would expect this problem after 250K or when the car is over 10 years old. My Godmother had a 1998 Accord that she had for over 20 years and it only needed a fuel pump as a major repair. A testament to Honda’s rock solid reliability. What happened?

Purchased car on July 7, 2021, that same day the car did not start when stepped on brake and pushed start button. Happened again on July 8, 2021. Called Dealer, Took it in on July 9, 2021. They could not duplicate problem. Assured me it was not a problem. Drove to CT on July 10, 2021, Did not start several times during the trip. On July 11, on return trip from CT a warning light came on dashboard. IN black letters in a red rectangular box - BRAKE. I called dealer on Monday July 12, 2021 and reported the warning. He said it meant there were cars in the way. The BRAKE IN A BOX occurred 2 more times (8/6 and 8/17). I called dealer who said it was not a problem. I subsequently searched the warning on line and it says "BRING YOUR CAR TO A DEALER. TO CONTINUE DRIVING COULD BE DANGEROUS!" I left the car with the dealer for 9 days. Sept 3 through September 11, 2021. They said they could not find anything wrong. I continue to have problems with starting the car, often getting the message that says TO START ENGINE: BRAKE+PUSH. Which is exactly what I do to start the car. When I picked up the car on September 11 (after 9 days with the dealer) the service manager told me he would contact their regional manager to report the problem to the regional manager. That's the last I heard from him except when I called and he said he had not heard from regional manager yet. I called again and left message but have not heard back. I have also submitted a complaint with HONDA and I have written 3 emails to the VP of Customer Relations. the VP has not responded to me at all. The complaint person contacted the dealer and dealer told him they could not duplicate the problem. After many weeks, complaint person contacted me to let me know this. He has no other recommendations. At this time i am afraid to drive the car and just drive it locally. I have avoided the highway, and any long trips because of my fear that there is something seriously wrong with the car and brakes.

The contact owns a 2021 Honda CR-V. The contact stated that the battery failed to start the engine. The vehicle was jumpstarted, and the vehicle was taken to the dealer. The contact noticed acid on top of the battery by the terminals and noticed that acid had dripped down the vehicle, and the battery was hot. The dealer replaced the battery. The contact stated that on a separate occasion, the battery short-circuited, and the vehicle failed to start. The vehicle was jumpstarted and taken to the dealer, where the battery was replaced. The battery failed a third time. The vehicle was taken to the dealer, where it was diagnosed that the A/C condenser had failed. The vehicle was repaired. Upon leaving the dealer, there was an abnormal sound coming from the vehicle. An unknown toxic gas emitted from the A/C vents and splashed the contact's face. The contact went to the emergency room and was informed that an unknown toxic gas had burned and injured the contact’s left eye. The contact was partially blind in the left eye. The contact took the vehicle back to the dealer, who diagnosed that the intake manifold had failed. The dealer stated that the intake manifold needed to be snapped back into place. The vehicle was repaired. The contact stated that the failure persisted, and abnormal sounds were coming from the vehicle. The vehicle was taken back to the dealer; however, the dealer refused to repair the vehicle and issued a no trespass order against the contact. The contact towed the vehicle to another dealer in Sarasota, Florida. The dealer installed a switch near the starter. The contact stated that afterwards it was found out that the dealer in Bradenton, Florida had failed to install the A/C filter. The A/C failure persisted. Several warning lights were illuminated. Most recently, after entering the parking lot at a gas station, there was an abnormally loud sound coming from the front end. Upon investigation, the contact noticed that the overflow of the coolant had leake
